A Comparative Study Of LVIS/LVIS Jr Stent And Enterprise Stent Assisted Coiling In The Treatment Of Intracranial Wide-necked Aneurysms

Objective:This retrospective study compared the incidence of complication,surgical outcome,and prognosis in the treatment of intracranial wide-neck aneurysms with Enterprise stents and LVIS/LVIS Jr stents.Methods:From July 2016 to July 2018,a total of 170 patients,178 aneurysms were included in the study,and patients were divided into Enterprise stent group and LVIS/LVIS Jr stent group according to the stent used during the treatment.The therapeutic effects,treatment-related complications,angiographic review and prognosis were compared retrospectively between the two groups.Results:The Enterprise stent group consisted of 64 patients with 70 aneurysms and the LVIS/LVIS Jr stent group consisted of 106 patients with 108 aneurysms.Immediately after surgery,the complete embolization and near-complete embolization aneurysm in the LVIS/LVIS Jr stent group was 103(95.37%),while the Enterprise stent group was 60(85.71%).There was a statistical difference between the two groups(P= 0.023).In the Enterprise stent group,9.23% of patients had treatment-related complications(1 procedure related hemorrhagic event,2 procedure related thrombosis events,3 postprocedure ischemic complications)and the LVIS/LVIS Jr stent group had an incidence of 6.60%.(4 procedure related hemorrhagic events,2 procedure related thrombosis events,1 postprocedure ischemic complications).The P values of procedure related hemorrhagic events,procedure related thrombosis events,postprocedure ischemic complications and overall complication rate were greater than 0.05 in the two groups,with no statistical difference.The progressive embolization in the LVIS/LVIS Jr stent group was 47.7%(21/44)higher than that in the Enterprise stent group(25.3%,8/32),and there was a statistical difference between the two groups.95.38% of the patients in the Enterprise stent group had a good mid-turn outcome(mRS 0-2),and 94.34% of the patients in the LVIS/LVIS Jr stent group had a good mid-turn outcome(mRS 0-2).There was no significant difference in the prognosis between the two groups(P=1.000).Conclusions:1.Both the Enterprise stent and the LVIS/LVIS Jr stent are safe and effective in the treatment of intracranial wide-necked aneurysms.2.LVIS/LVIS Jr stents have a higher rate of complete and nearly complete embolization than Enterprise stents,and a higher progressive embolization rate of LVIS/LVIS Jr stents during mid-term angiographic follow-up.3.The incidence of complications and favorable medium-term outcome were similar for the treatment of Enterprise stent and LVIS/LVIS Jr stent.
