Objective:The study compared the clinical treatment effect of LVIS and Solitaire AB stent-assisted embolization of intracranial anterior circulation of a wide carotid aneurysm.Method:Retrospective analysis Patients treated with LVIS or Solitaire AB stent-assisted coil embolization in 2016.01-2019.06 in Brain Hospital of Hunan Provincial, who were divided into LVIS stent group and Solitaire AB stent group.Chi-square test?Calibration chi-square test and Fisher accurate test were used to analyze the effect of two groups of stent-assisted anterior circulation of wide carotid aneurysm.Include:cerebral aneurysm embolization raymond grade immediately after operation and 6 months after contrast examination,Surgical complications, the prognosis of patients in 6 months after operation.Results:86 patients were included according to the admission criteria.Solitaire AB stent group included 27 patients and Solitaire AB stent-assisted embolization 29 aneurysms;LVIS stent group included 59 patients LVIS stent-assisted embolization 62 aneurysms. The immediate postoperative imaging showed that the Raymond grade I of the stent group was 16(55.17%)and that of the LVIS stent group was 38(61.29%).4 postoperative complications(14.81%)in the Solitaire AB stent group included,2 intraoperative aneurysm bleeding(7.41%),1 intraoperative acute thrombosis(3.70%),1 CVS/TIA/infarction(3.70%),and no postoperative bleeding events. In the LVIS stent group,7 patients(11.85%)had treatment-related complications, including 1 intraoperative aneurysm bleeding(1.69%),3 intraoperative acute stent bleeding(5.08%),1 postoperative bleeding(1.69%),2 cases(3.39%)of CVS/TIA/cerebral infarction after operation. There were no cases in both groups of patients with intraoperative coil prolapse, stent displacement and surgery-related death. The p value of intraoperative aneurysm hemorrhage, intraoperative acute thrombosis,CVS/TIA/cerebral infarction and overall complication rate were higher than 0.05 in the two groups, there was no significant difference. There were 19 aneurysms,12 aneurysms(63.16%)in Solitaire AB stent group,2 aneurysms(10.53%)recurred;32 aneurysms in lvis stent group,27 aneurysms(84.37%)were raymond I embolization, and 1 aneurysm(3.13%)relapsed.There was no significant difference in total embolization rate and recurrence rate between the two groups at 6 months review. The proportion of increased grade Raymond aneurysms in the LVIS stent group was higher than that in the Solitaire AB stent group(46.88%>15.79%)after 6 months angiography follow-up after treatment, and there was a statistical difference between the two groups(p=0.025).All patients received follow-up to assess prognosis,55 patients(93.22%)had a good prognosis in LVIS stents group,24 patients with stents(88.89%)had a good prognosis in Solitaire AB stent group, and there was no statistical difference in the rate of good prognosis between the two groups(p=0.797).Conclusions:1.Solitaire AB and LVIS stents are safe and effective in the treatment of intracranial anterior circulation wide carotid aneurysms.2.Solitaire AB stents complete embolization rate immediately after operation, perioperative complication rate, complete embolization rate?aneurysm recurrence rate and good follow-up prognosis rate at 6-month follow-up were similar to LVIS stent.3.The degree of complete embolization of the aneurysm at 6 months follow-up of the LVIS stent is higher to the Solitaire AB stent.
