Objective To assess comparative prognostic efficacy of various bone reconstruction type and different chemotherapy(either irregular or regular chemotherapy)respectively in the treatment of long bone primary malignant neoplasmas in the youths.Methods We collected 49 long bone primary malignant tumors patients who received extensive surgical resection in limb salvage from 2010 to 2017 in our institution,their age were between 11 and 30 with average age of 17.9±4.6.A comparative analysis of complication rate,local relapse rate,metastasis rate,long term survival,limb MSTS function was conducted between prosthesis group and pasturisation group or irregular group and regular group respectively.Results(1)Complication rate,local relapse rate,metastasis rate,long term survival showed no significant difference between prosthesis group and pasturisation group;(2)Pasturisation reconstruction could offer an better favourably limb MSTS function(3)Once long bone primary malignant tumors was carried out by extensive surgical resection,local curative rate cannot be improved by regular chemotherapy.(4)Metastasis rate and long term survival can be modified by effective and regular chemotherapy,Compared to irregular chemotherapy,the consequence exhibited significantly differently.Conclusions(1)When bone neoplasm was resected by en block and surgical margin showed no malignant cell,favorable local curative rate was achieved.(2)Various limb reconstruction technology can be selected for appropriate patient according to age of patient,tumor location,surgical status,biological action of tumor.(3)An decreased metastasis rate and improved long term survival can achieved by eradication of metastatic and micro lesions through completion of regular chemotherapy.
