Extracellular matrix is the nature screen between tumor cells and normal tissue of human body, it prevents tumor cells from pervasion. The composing of extracellular matrix is regulated by matrix metalloproteinases greatly. It is proved that tumor cells and fibroblasts around can produce matrix metalloproteinases, which is regulated by extracellular matrix metalloproteinase inducer. The content and activity of matrix metalloproteinases and extracellular matrix metalloproteinase inducer in malignant tumor cells are obviously higher than that in the normal tissue around, so they have become external and nicer molecular object for potentialof tumor metastasis, and hotspot in research for tumor invasion and metastasis. We use the immunohistochemistry (S-P) to study the expression of MMP-2, MMP-9 and EMMPRIN in clinical osteosarcoma specimens, and find that the male rate of MMP-2 expression is 76.67%, that of MMP-9 is 78.33% and the EMMPRIN is 71.67%. In use of the statistical analysis, we find that the expression of MMP-2, MMP-9 and EMMPRIN are closely correlated with local recurrence (P < 0.05). The expression of EMMPRIN is closely correlated with metastasis (P<0. 05). The coefficients of correlation between EMMPRIN and MMP-2 is 0. 626 (P<0. 05), and that between EMMPRIN and MMP-9 is 0.504 (P<0. 05). It is proved that MMP-2, MMP-9 and EMMPRIN can be used as the early period factor in tumor metastasis and local recurrence. To detect the expressions can guide us in the treatment for the .patients with osteosarcoma in the future.Bone malignant tumor is often occurred in youths and childrens, it is strongly invasive and has more deformity. The medicals attach importance to it for its early metastasis to lung and other organs, and easily recurrence after surgical excision. Limb salvage surgery is the major surgical therapy for bone malignant tumor at present, through which the patients get better functional resumption. How to prolong patient's survival become more important now. The primary definition for adaption oflimb salvage surgery is stage I, stage IIA, and stage IIB which is sensitive to medicine therapy. The tumor has no invasion of major blood vessel and nerve. Bone malignant tumor of stage IIB has strong local invasion, so how can limb salvage surgery be safely taken is hotspot at present. We use the immunohistochemistry (SABC) to study the expression of EMMPRIN and the expression of alkaline phosphatase in clinical specimens of bone malignant tumor of stage IIB, and find micro-metastasis, which proves the local infiltration and invasion of bone malignant tumor of stage IIB. The high expression of EMMPRIN has been detected, this is closely accorded to the opinion that the EMMPRIN accelerates tumor's local infiltration and invasion. For the existence of micro-metastasis, we improve surgical method including disposing theca of blood vessel and dunking the extension of surgery with distilled water, which achieves approving result.
