Objective To evaluate the outcome and curative effect of extremity primary malignant bone tumors with different methods of reconstructive surgery.Methods From January 2010 to December 2015,the clinical data of 158 patients with extremity primary malignant bone tumors were analyzed retrospectively.Only 126 patients were followed up to the end and 32 of them were lost.Among all of the 126 follow-up patients,there were 65 males and 61 females aged from 5 to 81 years old,with an average age 28.3±16.9 years old.Of all the 126 primary malignant bone tumors,62 occurred on the left side and 64 on the right side.There are 91 cases of osteosarcoma,15 of chondrosarcoma,7 of Ewing's sarcoma,4 of fusocellular sarcoma,7 of malignant giant cell tumor of bone and 2 of undifferentiated pleomorphic sarcoma.Tumors found in 66 patients were in the femur,40 in the tibia,16 in the humerus and 4 in the radius.According to the ways of surgery,it was divided into 2 groups named biological reconstruction group(group A)and mechanical reconstruction group(group B)respectively.There were 43 patients in the group A while 83 in the group B.Results of the recurrence rate,metastasis rate,complication,and MSTS postoperative functional scores were compared between the groups.Results 126 patients were followed up from 10 to 67 months with an average of 38.5±11.2 months.There were no significant differences between two groups in recurrence rate,metastasis rate,complication after surgery(P>0.05).The MSTS postoperative functional scores of group A were better than those of group B(P<0.05).Conclusion Individualized managements toward extremity primary malignant bone tumors should be considered concerning patients' specific conditions,such as the pathological type,clinical staging,size,location of tumor and the age of patients.Compared with mechanical reconstruction,patients undergone biological reconstruction showed better postoperative long-term extremity function.
