ObjectiveTo introduce the method of epicanthus correction by modified transverse suture, and to record the preoperative and postoperative data of eye shape measurement,so as to discuss the clinical effect of epicanthus correction by modified transverse suture.Methods32 patients with epicanthus who were admitted to plastic and cosmetic surgery department of our hospital from October 2007 to October 2018 and were successfully followed up for 6 months to 1 year after surgery were selected as the research objects, including 2 male patients and 30 female patients. In 32 patients, modified transverse suture epicanthus correction was performed, and double eyelid surgery was performed in some patients during the same period.Preoperative and postoperative tarsal fissure length(HLFL),epicanthus distance(MED),and epicanthus distance(HCD)of each patient were accurately measured and recorded, intraoperative scar was evaluated, and the ratio of tarsal fissure length to epicanthus distance was calculated. The improvement of the preoperative and postoperative data, the length of palpebral fissure and the distance between epicanthus(HLFL/MED),was analyzed by IBM SOSS 23,and the correction effect of epicanthus was evaluated. Results 32 patients were followed up for 6 months to 1 year after surgery,an average of 9.4 months. All patients were healed in stage I after surgery, with good canthus shape,no surgical scar, and complete lacrimal caruncle exposure. The distance between epicanthus canthus was 37.98 3.68 mm,33.01 3.08 mm,34.17 2.59 mm before surgery,32.38 2.33 mm,33.19 2.61 mm immediately after surgery and 6 months after surgery, respectively. The distance between epicanthus canthus was 35.02 2.53 mm,32.38 2.33 mm,33.19 2.61 mm,respective. The length of palpebral fissure was 26.98 1.01 mm,3.11 1.53 mm and 28.62 1.01 mm respectively before, immediately after and 6 months after surgery. The HLFL/MED ratios before, immediately after and 6 months after surgery were 0.752 0.065,0.948 0.068 and 0.842 0.055,respectively.Compared with preoperative, epicanthus distance was significantly shortened within 6 months after surgery, with the shortened distance of 3.38 1.21 mm,P < 0.05,and the difference was statistically significant.Canthus distance significantly increased within 6 months after surgery compared with immediately after surgery, with an added value of 2.19 0.83 mm,P < 0.05,indicating a statistically significant difference.Compared with the preoperative, the canthal distance within 6 months after surgery was significantly shortened by 0.93 0.62 mm,P < 0.05,and the difference was statistically significant.Compared with the postoperative, the canthal distance was significantly increased within 6 months after the surgery, and the added value was 1.02 0.68 mm,P < 0.05,and the difference was statistically significant.Compared with that before surgery, the length of palpebral fissure was significantly increased 6 months after surgery, with an added value of 1.81 0.46 mm,P < 0.05,indicating a statistically significant difference.Compared with the immediate postoperative period, the palpebral fissure length was significantly reduced at 6 months after surgery, with a decrease value of 1.29 0.73 mm,P < 0.05,indicating a statistically significant difference.Compared with that before surgery,HLFL/MED increased significantly at 6 months after surgery, with an added value of 0.119 0.025,P < 0.05,indicating a statistically significant difference.Compared with the immediate postoperative period, there was a significant decrease in HLFL/MED at 6 months after surgery, with a decrease value of 0.098 0.041,P < 0.05,indicating a statistically significant difference. The PSAS score of patients and OSAS score of observers were compared with POSAS after surgery. The postoperative POSAS scores of PSAS and OSAS were 10.35 0.76 and 7.21 0.73,respectively,and the overall satisfaction scores of patients and OSAS were 2.21 0.19 and 2.59 0.28,respectively.Conclusionthe improved transverse suture method has the advantages of easy operation and less trauma,so the improved transverse suture method can achieve a significant effect in the correction of epicanthus, and patients with postoperative scar or recurrence of epicanthus and other complications are worthy of clinical application.
