ObjectiveTo evaluate the effectiveness of acupuncture in the treatment of migraine without aura,and to determine the specificity of the therapeutic effect of acupuncture by comparing whether the traditional acupuncture group(acupuncture group)is superior to the placebo acupuncture group(sham acupuncture group)in improving the symptoms,life and work disability index of migraine.MethodAccording to the method of randomized controlled study,60 patients with migraine without aura who met the diagnostic criteria were randomly divided into acupuncture group and sham-acupuncture group,30 in each group.Acupuncture group: main points: Hegu(bilateral),Taichong(bilateral),Sun(unilateral),Ratgu(unilateral),Fengchi(unilateral);Acupoints: Yangming meridian:scalp(unilateral);Sun meridian: Tianzhu(unilateral);Jueyin meridian: Baihui(unilateral);Operation methods: supine position,skin routine disinfection.NO.16 acupuncture needle produced by asia-med company in Germany was used.After needling,the needle was lifted,inserted and twisted three times in a small range evenly at each acupoint.The needle was retained for 30 minutes.The needle was lifted,inserted and twisted once in a small range evenly every10 minutes,and the needle was twisted four times in a row.Four weeks as a cycle,once every other day,treatment 10 times,rest9 days after the start of the next cycle of treatment.The treatment lasted for 8 weeks and 20 times.Acupoint selection in sham-acupuncture group: eight stimulation points were selected to locate in neck and shoulder: according to National Standard of the People's Republic of China(GB/T 12346-2006),"Name and Location of Acupoints" to avoid acupoints.Stimulation point 1: midpoint(bilateral)of shoulder well and mega-bone connection;stimulation point 2: 5 inches(bilateral)below the seventh thoracic spinous process;stimulation point 3: 5 inches(bilateral)below the eighth thoracic spinous process;stimulation point 4: 5 inches(bilateral)below the ninth thoracic spinous process.Operation method: supine position,skin routine disinfection.Streitberger needle produced by asia-med company in Germany was used.After needling,the needle was lifted,inserted and twisted three times in a small range evenly at each acupoint,and the needle was retained for 30 minutes.Every10 minutes,the needle was lifted,inserted and twisted once in a small range evenly,and the needle was twisted four times in a row.Frequency,course and time of treatment were the same as those of the traditional acupuncture group(even if patients achieved clinical cure during treatment,they still need to finish 2 cycles of acupuncture treatment).The total duration of migraine attack every 4 weeks,the total score of pain intensity and the score of disability assessment of migraine were compared between the two groups during the treatment period and the follow-up period of three months after treatment.The therapeutic effects of the two groups were evaluated.The data were processed and analyzed by SPSS21.0software.Results1.After 5-8 weeks of treatment and 17-20 weeks of follow-up,the effective rate of the acupuncture group was significantly higher than that of the sham-acupuncture group(P < 0.05),but there was no significant difference between the two groups after 1-4 weeks of treatment(P > 0.05).2.The total duration of migraine pain in the acupuncture group decreased significantly at each node compared with the baseline period(P < 0.01);compared with the sham-acupuncture group,except for the treatment period of 1-4 weeks,the total duration of attack in the other node-acupuncture group was significantly less than that in the sham-acupuncture group,and the total duration of pain tended to be stable,with statistical difference between the two groups(P < 0.01).3.The total score of pain intensity(VAS)of migraine in the two groups decreased in different degrees every 4 weeks.The decrease of pain intensity in the acupuncture group was more obvious than that in the sham acupuncture group during the treatment and follow-up period,and there was statistical difference between the two groups(P < 0.05).4.Migraine disability assessment score(MIDAS)in the acupuncture group was significantly lower than that in the baseline period(P< 0.01),while there was no significant decrease in the sham-acupuncture group(P > 0.05).ConclusionAcupuncture can effectively reduce the duration of migraine attacks,reduce pain intensity,improve the degree of disability,long-term efficacy is better than sham acupuncture,and less adverse reactions,high safety factor,it is worthy of clinical application.
