Study On The Effect Of Acupuncture On The Integrity Of White Matter Fibers In Patients With Migraine Without Aura

OBJECTIVE:As a common primary headache,migraine has a high morbidity rate and repeated attacks,which brings great pain and annoyance to patients.Previous studies have confirmed the clinical efficacy of acupuncture in the treatment of migraine.Domestically,more mature guidelines for acupuncture and moxibustion for migraine have been formulated and widely used in clinical practice.Diffusion tensor imaging(DTI)technology can show the white matter fiber trend in the living body without harming the examiner.The fractional anisotropy(FA)is one of the important indicators for evaluating the structural integrity of white matter fiber in DTI.Automating fiber-tract quantification(AFQ)is a new analysis method for processing DTI data.It can calculate the parameter changes of 100 sampling points on the 20 main fiber bundles of the whole brain for quantitative analysis,which is more refined than other DTI data analysis methods.METHODS:Taking migraine without aura patients as the research object,using diffusion tensor imaging technology and automating fiber-tract quantification analysis method,observe and compare the changes of white matter fiber structure in migraine without aura patients and healthy control group,and before and after acupuncture migraine without aura patients The changes of white matter fiber,to explore the effect of acupuncture on the white matter of migraine without aura patients,and to reflect the anxiety,depression and sleep disorders of migraine without aura patients before and after acupuncture through the scale,and provide neuroimaging evidence support for acupuncture treatment of migraine without aura.RESULTS:Compared with healthy controls,FA values of migraine without aura patients decreased significantly in the left thalamic radiation beam,left corticospinal cord bundle,left cingulum cingulate,bilateral cingulum hippocampus,callosum forceps major,lower left In frontal occipital bundle,left upper longitudinal bundle,and right arcuate bundle(P<0.05),FA value of left uncinate bundle increased significantly(P<0.05).Further comparing the FA values of 100 nodes on 20 white matter fibers of migraine without aura patients and healthy controls,the FA values of migraine without aura patients decreased significantly in the right corticospinal tract(nodes 39-45)and right arcuate tract(nodes 1-5)Some nodes of the callosum forceps minor,bilateral inferior forehead occipital bundles,and right uncinate bundles have significantly decreased FA values.Compared with patients before acupuncture,the value of FA of migraine without aura patients after acupuncture was significantly higher on the right side of the hippocampus,right lower forehead occipital bundle,left upper longitudinal bundle,right uncinate bundle,and bilateral arcuate bundles(P<0.05),The FA value of the left hippocampus significantly decreased(P<0.05).Further comparing patients' the FA values of 100 nodes on 20 white matter fibers before and after acupuncture,the FA values of migraine without aura patients after acupuncture increased significantly in the right corticospinal tract(nodes 33-46,56-59)and right arcuate tract(nodes 1-11,52-53,90-93),the left cingulum cingulate(nodes 53-56,63-66,)in the right thalamus radiation beam,corpus callosum small forceps,bilateral lower The FA value of some nodes in frontal occipital bundle,right lower longitudinal bundle,left upper longitudinal bundle,right uncinate bundle,and left arcuate bundle increased significantly,and FA values in some nodes of the callosum forceps major and minor decreased significantly.Compared with before acupuncture,the number of headache days,VAS score,PSQI,and SDS of Migraine without aura patients were significantly reduced(P<0.05),but the headache frequency(P=0.062)and SAS(P=0.155)were not significantly reduced after treatment.CONCLUSIONS:Compared with the healthy control group,MIGRAINE WITHOUT AURA patients have white matter fiber damage,and acupuncture can promote the recovery of damaged white matter in patients with migraine without aura.At the same time,the number of headache days,degree of headache,sleep and depression of Migraine without aura patients with acupuncture has improved.
