Clinical Observation And The Observed Results Under Dermatoscope Of Guizhi And Longgu Muli Decoction With Erzhi Pill Combination In The Treatment Of Alopecia Areata

Objective: To observe the clinical efficacy and clinical safety of Guizhi and Longgu Muli Decoction with Erzhi pill Combination in the treatment of liver and kidney deficiency of alopecia areata.The density terminal measured by dermoscope and dermoscopic features were taken as the clinical efficacy observation index,which provided a clinical basis for the promotion and application of Modified Guizhi and Longgu Muli Decoction with Erzhi pill Combination in the treatment of alopecia areata and application of dermoscope to determine the therapeutic effect of alopecia areata.Methods: A total of 64 cases of alopecia areata patients who met the inclusion criteria were randomly divided into the treatment group and the control group.There were 3 cases of detachment during the experiment and remained 61 effective cases.The patients in the treatment group(30 cases)were treated orally with Guizhi and Longgu Muli Decoction with Erzhi pill Combination while the control group(31 cases)received the Compound Glycyrrhizin Tablet.After 12 weeks of treatment,the clinical effects of the two groups were observed.The microscopic effects were observed at 0 weeks,4 weeks,8 weeks and 12 weeks by dermatoscope.The clinical safety of the drugs was evaluated according to the changes of liver function,renal function,blood pressure and blood potassium before and after treatment.Finally,all data were statistically analyzed by SPSS 22.0 to determine the effects.Results:1.Clinical effects: After three courses(12 weeks)of treatment,in the treatment group,the total effective rate was 93.33% with 16 cases completely cured,8 cases significantly effective,4 cases effective,while in the control group,the total effective rate was 74.19% with 7 case completely cured,13 cases significantly effective,3 cases effective.The total effective rate of the treatment group was significantly higher than that of the control group(P<0.05).2.After microscopic observation of dermatoscopy,the difference of density terminal at 0 weeks,4 weeks,8 weeks and 12 weeks was statistically significant(P＜0.05).With the progress of treatment,the density terminal of both groups increased.The difference of the density terminal between the treatment group and control group at 4 weeks,8 weeks,and 12 weeks was statistically significant(P?0.05).The increase of the density terminal on the treatment group was better than that of the control group.The results are consistent with the results observed by the eyes after 12 weeks.3.The prevalence of black dots,broken hairs and exclamation mark hairs was negatively correlated with the duration of alopecia areata.There was no significant difference in the total scores of the three types of dermoscopic features between the two groups before treatment(P>0.05).After 4 weeks of treatment,the difference of total scores of the three types of dermoscopic features between the two groups was statistically significant(P＜ 0.05),and the reduction or disappearance of dermoscopic features caused by the treatment group was more obvious.4.In the control group,there were 2 cases of adverse reactions mainly including increased blood pressure and eyelid edema.However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).The incidence of adverse reactions in the treatment group was 0,The safety of Guizhi and Longgu Muli Decoction with Erzhi pill Combination deserves recognition.The safety evaluation of the two groups of drugs remains to be further observed in large sample experiments.Conclusion:1.Guizhi and Longgu Muli Decoction with Erzhi pill Combination is effective and safe on treating alopecia areata with the pattern of liver and kidney deficiency.It provides a new method for the treatment of liver and kidney deficiency alopecia areata.2.Guizhi and Longgu Muli Decoction with Erzhi pill Combination has a better therapeutic effects than compound glycyrrhizin tablets in treating alopecia areata with the pattern of liver and kidney deficiency.3.Non-invasive dermoscopy can make an early objective judgment on the therapeutic effect of alopecia areata,which is worthy of clinical promotion and application.
