Preliminary Study On The Curative Effect Of Traditional Chinese Medicine Combined With Non-ablative Fractional Laser In The Treatment Of Alopecia Areata With Liver And Kidney Deficienc

Objective:The etiology of alopecia areata is unclear,the conventional treatment cannot meet the needs of clinic practice.Laser has got many curative effect in alopecia areata.In this study,traditional Chinese medicine was combined with the 1565nm nonablative fractional laser for the treatment of alopecia areata.Try to find a combined treatment for alopecia areata and evaluate the value of dermoscopy in the therapeutic effect for alopecia areata.Methods:30 patients who met the diagnostic criteria were randomly divided into the treatment group and the control group.Each group has 15 patients.In the control group,patients only used WuDiShengFaWan for 8 weeks,40 pills each time,3 times a day.In the treatment group,1565nm nonablative fractional laser treatment was given on the basis of oral medicine.The treatment energy was 40mJ,150ub/cm2,and each treatment period was 2 weeks.The full course of treatment was 2 months.Dermoscopy photos were taken before the treatment and at the second week,the fourth week,the sixth week and the ten week to record the mean hair density terminal、hair density vellus and hair density,as well as the mirror features of dermoscopy including yellow spot sign,black spot sign,exclamation mark hair and broken hair.Finally,all data were statistically analyzed by SPSS 22.0 to estimate the therapeutic effect.Results:1.By visual observation,the effective rate of the treatment group was 93.33%,Among the 15 patients,2 were cured,6 were effective,5 were effective and 4 were ineffective.The total effective rate of the treatment group was higher than control group,but the difference in clinical efficacy between the two groups was not statistically significant(P>0.05).2.The hair density terminal、hair density vellus and hair density was significantly increased after the treatment(P<0.05),The growth of hair density terminal and total hair density in the teratment group was significantly higher than the control group(P<0.05).The total hair density in the treatment group was significantly higher than that in the control group from week 6(P<0.05).The curative effect of the treatment group was significantly better than that of the control group judging by dermoscopy.3.Among the 30 patients with alopecia areata,the skin mirror features before treatment were 66.67%yellow dot,73.33%black dot,30%exclamation mark,and 63.33%broken hair,which significantly disappeared after treatment.After 10 weeks of treatment,they were respectively 20%yellow dot,20%black dot,0%exclamation mark,and 16.67%broken hair.At 6 week,the exclamation marks in the two groups was completely disappeared,and at 10 week,the yellow dots in the treatment group was completely disappeared.Conclusion:1.The efficacy of the treatment group was significantly better than the control group,WuDiShengFaWan combined with 1565nm nonablative fractional laser is worthy of further application in clinical practice.2.The occurrence rate of yellow dot,black dot,break hair and exclamation mark hair is high in the scalp of alopecia areata patients.With the recovery of disease,they decreased gradually and disappeared.3.Non-invasive dermoscopy can make an early objective judgment on the therapeutic effect of alopecia areata,and provide objective hair data,which is worthy of clinical application.
