Clinical Observation Of Huanglian Wendan Decoction Combining With Low-frequency Repetitive Transcranial Magnetic Stimulation In The Treatment Of Insomnia Induced By Phlegm-heat Internal Disturbance

Objective:Observe the clinical effect of huanglian wendan decoction combined with low frequency(1Hz)repetitive transcranial magnetic stimulation(rTMS)in the treatment of insomnia induced by phlegm-heat internal disturbance.Methods:In this study,120 patients with phlegm-heat internal disturbance insomnia meeting the inclusion criteria are randomly divided into3 groups,namely,transcranial magnetic group,traditional Chinese medicine group and combined group,with 40 cases in each group.All patients in the three groups received basic medical treatment,and the TMS group is treated with low-frequency(1Hz)rTMS.TCM group is treated with huanglian wendan decoction plus.The combined group is treated with huanglian wendan decoction combined with low-frequency(1Hz)rTMS.The duration of treatment is 10 days,and the pitzburg sleep quality index(PSQI)and TCM syndrome score are used as the observation indexes.The three groups of patients are evaluated before and after treatment to compare the efficacy.Results:1.Case inclusion and final completionIn this study,a total of 120 patients with insomnia who met the inclusion criteria are included.They are randomly divided into three groups with an average of 40 cases in each group.Finally,113 patients completed clinical observation,with a total of 7 cases falling off,with a total falling off rate of 5.83%.Among them,4 cases fell off in the transcranial magnetic group,and 36 cases finally completed.In the TCM group,2 cases fell off and 38 cases finally completed.One case fell off in the combined group,and 39 cases finally completed.2.General situationThrough statistical analysis,there is no significant difference in gender,age and course of disease among the three groups(P >0.05).All three groups are middle-aged and elderly patients,female than male,subacute,chronic course.3.TCM syndrome pointsThere is no significant difference in TCM syndrome points between the three groups before treatment(P > 0.05),while there is significant difference in TCM syndrome points after treatment(P< 0.05).All three groups could reduce TCM syndrome points,among which the combined group had the lowest TCM syndrome points after treatment,followed by the TCM group,and finally the transcranial magnetic group.4.Efficacy of TCM syndromesAll three groups are effective in improving TCM syndromes.The combined group had better efficacy than the traditional Chinese medicine group and the traditional Chinese medicine group hadbetter efficacy than the transcranial magnetic group.5.PSQI scoreThere is no significant difference in PSQI score before treatment in the three groups(P > 0.05),and there is a significant difference in PSQI score after treatment in the three groups(P < 0.05).PSQI score in all three groups could be reduced.PSQI score is the lowest in the combined group after treatment,and there is no significant difference between the traditional Chinese medicine group and the transcranial magnetic group after treatment.6.Clinical efficacyThe total effective rate of the TMS group is 75.00%,that of the TCM group is 78.95%,and that of the combined group is 92.31%.All three groups can effectively treat insomnia,among which the combined group has the best clinical efficacy,and the traditional Chinese medicine group and the transcranial magnetic group have no significant difference in efficacy.7.Safety evaluationDuring the study,the vital signs of the patients in the thre e groups are stable,and there are no significant changes in bl ood routine,liver and kidney function,electrolyte and electroca-rdiogram.No adverse reactions are observed in the three group s suggesting that the study is safe.Conclusion:Huanglian wendan decoction combined with low-frequency(1Hz)repeated transcranial magnetic stimulation(rTMS)can effectively treat internal disturbance of phlegm and heat insomnia,improve sleep quality and TCM syndromes without obvious adverse reactions,with high safety.It is worthy of promotion and application.
