Clinical Observation On Effect Of Retention Enema With Pulsatilla Decoction On Immune And Coagulation Function Of Damp-heat Ulcerative Colitis (Rectal Type)

Objective:To observe the clinical effect of retention enema with Pulsatilla Decoction on damp-heat ulcerative colitis(rectal type)and its influence on immune and coagulation functions.Method: Sixty-one patients with damp-heat UC(rectal type)diagnosed by traditional Chinese and Western medicine from the outpatient and inpatient in anorectal department 2 of the first affiliated hospital of Anhui university of Chinese medicine from December 2017 to December 2018 were randomly selected,including 31 patients in treatment group and 30 patients in control group.The treatment group was treated by retention enema with Pulsatilla Decoction.The control group was given mesalazine suppository to accept anus.The changes of TCM symptom score,disease activity score,submucosal image score under colonoscopy,peripheral blood serum immunoglobulin(IgG,IgA,IgM),complement system(C3,C4),blood cytokines(IL-8,IL-13),platelet count(PLT),coagulation index(FBG,PT,D-dimer)and overall clinical efficacy were observed before and after 8-week of treatment.Result: The TCM symPTom score,disease activity score,submucosal image score under colonoscopy,immunoglobulin IgG,IgA,complement C3,C4,IL-8,FBG,pt,Ddimer coagulation indexes of the treatment group and the control group were significantly lower than those without treatment(p<0.05).The levels of IL-13 in the two groups were significantly higher than before,and the difference was statistically significant(p<0.01).There was no significant difference in immunoglobulin IgM and PLT between the two groups(p>0.05).After 8 weeks of treatment,the improvements of TCM symPTom score and disease activity score in the treatment group are better than those in the control group(p<0.05).FBG and pt in the treatment group are significantly lower than those in the control group(p<0.05).there are no significant differences in mucosal image score,immunoglobulin IgG,IgA,complement C3,C4,IL-8,IL-13 and D-D index changes between the two groups under colonoscopy(p>0.05).The clinical effect of the treatment group is better than that of the control group(p<0.01).No adverse reactions were found in the treatment group,2 cases of rash and 1 case of stomach discomfort accompanied by nausea were present in the control group.Conclusion: Retention enema with Pulsatilla Decoction can improve clinical symptoms of patients with damp-heat ulcerative colitis(rectal type),repair damaged intestinal mucosa,and enhance immune and coagulation functions of UC(rectal type)patients by reducing IgG,IgA,C3,C4,IL-8,FBG,PT and D-D levels,increasing IL-13 levels.Moreover,the retention enema with Pulsatilla Decoction had no significant side effects in the treatment of patients with damp-heat ulcerative colitis(rectal type).
