Driven by various exploration reform measures,the medical service industry has achieved rapid development.However,over the years,the problem of "seeing a doctor hard and seeing a doctor expensive" did not seem to have been significantly improved.Controlling medical expenses has always been the focus of government medical reform.How to effectively control the cost of medical treatment is controversial in academia,the difference lies in the control of medical expenses is the government regulation or the introduction of market competition.The purpose of this paper is to discuss the institutional arrangements for controlling medical expenses by analyzing the effect of government medical cost control and promoting the development of private medical institutions to curb the rising of medical expenses.On the one hand,this paper can enrich the research on medical expense control mechanism,on the other hand,it provides the theoretical basis and empirical data for the medical reform of the government.This article mainly uses two kinds of research methods:First,comparative analysis.Analyze the operational efficiency of public medical institutions and private medical institutions to explore which kind of business model is more suitable for controlling medical expenses;Second,the combination of qualitative analysis and empirical analysis.Through the qualitative analysis of the impact of the development of private medical institutions on the medical service industry,and we use the objective data to quantitatively verify the relationship between the development of private medical institutions and the control of medical expenses.After Huasman test,the empirical model adopts the fixed coefficient of effect variable coefficient model of panel data in the econometrics for regression analysis.This article mainly studies the effect of government regulation on medical expenses and the effect of introducing market competition to control medical expenses by promoting the development of private medical institutions.The main research contents include:(1)Analysis of the characteristics of the medical service market.Through the analysis of information attributes,supply and demand attributes and market structure characteristics of medical services,we find that medical service market is characterized by asymmetric information,less demand elasticity,supplier-oriented and administrative monopoly.The complexity of the market for health services is a justification for the government regulating the cost of health care services;(2)Analyzes the effects of government regulation of medical costs.The increasing demand for medical services and the administrative monopoly of the medical service market have led to an upward trend in medical expenses.Although the government has taken various control measures to control medical expenses,it is hard to fundamentally curb the rising trend of medical expenses and cause"failure" of government regulation.;(3)comparative analysis of public medical institutions and private medical institutions operating efficiency.Based on the theory of property rights and principal-agent theory,it is found that the operating efficiency of private medical institutions is significantly higher than that of public medical institutions.And then analyze the status of development of private medical institutions and its constraints,policy restrictions,talent constraints,reputation and other restrictions on the development of private medical institutions is the main factor;(4)the use of 2010-2015 31 provinces,autonomous regions and municipalities in China empirical data analyze and verify the impact of the development of private medical institutions on medical expenses.The main conclusions of this paper include:(1)The complexity of medical service transactions is the reason why the government controls the medical expenses.The medical industry is related to the life and health of people.The government needs to ensure the accessibility of medical services,guarantee people's basic rights and interests,and then control the medical expenses;(2)It is difficult for government regulation to effectively control medical expenses.Government regulation can not solve the contradiction between supply and demand and restrain medical institutions from inducing demand behavior.Instead,government regulation stimulates medical institutions to induce demand and leads to the rise of patients' medical costs;(3)Introducing more efficient private medical institutions into the medical field is the precondition for effective control of medical expenses.The complete and clear ownership of property,the short relationship between principal and agent helps to form an effective supervision and incentive to promote private medical institutions to improve service efficiency and quality,reduce the cost of medical services;(4)Some government-based discriminatory policies inhibit the development of private medical institutions.Government approval process is complicated,tax items and other policies to restrict the development of private medical institutions;(5)private medical institutions into the field of medical services help to curb rising medical costs.The competition in medical service market helps to spread medical information,alleviates the problem of information asymmetry,gives full play to the market price discovery function,and helps the healthy development of medical service market.According to the conclusions of the study,the policy implications proposed in this paper include:First,to promote property rights reform in public medical institutions and improve service efficiency;secondly,to further open up medical services to private medical institutions to curb medical expenses through competition;thirdly,Relax the cost of medical services control;Fourth,the government clearly the role of positioning,improve laws and regulations,and strengthen quality control.
