BackgroundAortic dissection is a common critical disease in vascular surgery.It has the characteristics of high mortality,rapid progress,high cost of treatment,high risk of operation and poor prognosis.The occurrence of this disease often brings great economic burden,psychological burden and social pressure to the family members of patients.In addition,the influence of other factors can easily lead the family members of patients to show negative emotions,such as anxiety and depression,which will affect the care of patients,and also affect the physical and mental health of the family members of patients.Nowadays,more and more attention has been paid to the psychological concerns of family members at home and abroad.Improving the psychological self-management ability of family members is a horizontal extension of patient care service,which belongs to the category of continuing nursing service and meets the needs of nursing development.ObjectiveTo understand the anxiety and depression of family members of patients with aortic dissection,analyze the influencing factors of anxiety and depression,and explore the effect of sympathetic nursing intervention on anxiety and depression of family members of patients with aortic dissection.MethodFamily members of 120 patients with aortic dissection who met the inclusion criteria in three tertiary hospitals in Anyang City from September 2017 to August 2018 were included in the study by convenience sampling method.Self-made questionnaire was used to investigate the basic information of family members of patients.Zung Self-rating Anxiety Scale(SAS)and Zung Self-rating Depression Scale(SDS)were used to investigate the anxiety and depression status of family members of patients with aortic dissection.The influencing factors of anxiety and depression were analyzed.63 family members with anxiety and depression were randomly divided into control group(31 cases)and experimental group(32 cases).The control group received routine nursing care: introduction of admission instructions,Department environment,safety information,regular visits,routine education of diseases;the experimental group was given sympathetic nursing intervention on the basis of routine nursing,including: arranging exclusive escort room.Creating a sympathetic environment,arranging visiting time and time reasonably,introducing "sympathizers" with similar experiences,etc.Statistical software SPSS21.0 was used.The statistical methods include descriptive statistical analysis,t test,?~2 test and Logistic regression analysis.Result1.Among 120 family members with aortic dissection,22 were simply anxious,18 were simply depressed and 23 were anxious and depressed.There were 63 family members of patients with anxiety and depression,accounting for 52.50%?There were 57 family members of patients without anxiety and depression,accounting for 47.50%?The average score of anxiety was(50.72±13.97),The average score of depression was(52.33±12.15),higher than the domestic norm.2.Gender(?~2=16.457,P<0.05)? number of caregivers(?~2=9.360,P<0.05)? conscious health status(?~2=14.603,P<0.05)?residence(?~2=24.902,P<0.05)?payment method(?~2=15.179,P<0.05)?education level(?~2=40.074,P<0.05)?occupational status(?~2=29.083,P<0.05)?monthly income(?~2=12.912,P<0.05)?knowledge of disease(?~2=8.087,P<0.05)?and type of dissection(?~2=51.841,P<0.05).There was a significant correlation between anxiety and depression of family members of patients with aortic dissection.The factors with statistical differences were taken as independent variables in the above-mentioned single factor analysis,and whether the family members of patients with aortic dissection had anxiety and depression was taken as dependent variables.Multivariate logistic regression analysis was adopted.The results showed that the low level of disease awareness,self-conscious and unhealthy state,the small number of caregivers and Stanford A type of patients with aortic dissection were the main risk factors of anxiety and depression in family members.3.Anxiety score of control group before intervention(68.68±6.52),depression score(67.67±6.74),anxiety score of the experimental group(67.16±6.41),The depression score was(66.34±6.68),There was no significant difference in anxiety and depression scores between the two groups.After intervention,the anxiety score of the control group was(55.68±5.49),depression score(54.68±5.32),and in the experimental group anxiety score(45.28±5.47),and depression score(46.41±5.28).The anxiety and depression scores of the two groups after intervention were significantly lower than those before intervention,and the difference was statistically significant(P < 0.05).Comparing the anxiety and depression scores of the two groups after intervention,the experimental group was significantly lower than the control group,and the difference was statistically significant(P < 0.05)?Conclusion1.The degree of anxiety and depression of family members of patients with aortic dissection is higher than that of domestic norm,and the gender,number of caregivers and self-consciousness are also higher.Health status,place of residence,mode of payment for expenses,educational level,occupational status,monthly income,knowledge of diseases,The type of dissection was correlated with other factors.The risk factors of anxiety and depression in family members are self-conscious unhealthy state,small number of caregivers,insufficient awareness of disease,and Stanford S type of patients.2.Empathy nursing is ideal in improving anxiety and depression of relatives of patients with clinical aortic dissection.
