Clinical Observation On Treatment Of Type 2 Diabetes Mellitus With Depression Disorder By The Method Of Shugan Jianpi And Yangyin Anshen

Objective:To observe the clinical efficacy of soothing liver and invigorating pleen and nourishing yin and tranquilizing mind in the treatment of type 2 diabetes mellitus complicated with depressive disorder,and to explore its mechanism.Method:Sixty-six outpatients and inpatients of Endocrinology Department of Heilongjiang Academy of Traditional Chinese Medicine met the diagnostic criteria of type 2 diabetes mellitus with depressive disorder.They all met the criteria of FPG<10.0 mmol/L,2hPG<13.0 mmol/L,HbA1c<9.0%,and 7<HAMD<17.They were randomly divided into two groups,33 cases in the treatment group and 33 cases in the control group.All patients were given basic treatment of diabetes;while the control group was given basic insulin + metformin + sertraline hydrochloride(5 0 m g each time,once a day,after breakfast).The treatment group was given basic insulin + netformin + soothing liver and strengthening spleen,nourishing yin and tranquilizing spirit Chinese herbal decoction(150ml each time,1 dose daily,warm before breakfast and half an hour after dinner).The course of treatment was 8 weeks.The changes and safety of FPG,2hPG,HbAlc.HAMD-17 score and reduction rate.HAMD-17 factor score,SDS score and reduction rate,TCM clinical symptoms(total score,single score,TCM curative effect index)were observed before and after treatment in the two groups.Result:1.After treatment,FPG,2 h P G and HbA1c in the two groups were lower than those before treatment,and the treatment group was better than the c?ntrol group(P<0.05).2.After treatment.HAMD-17 score.SDS score and HAMD-17 factor score of the two groups were lower than those before treatment,and the treatment group was better than the control group(P<0.05).The treatment group was superior to the control group in improving anxiety,somatization,sleep disorders and weight loss(P<0.05).3.Af t e r treatment,the total integral and single integral of TCM syndromes in the two groups were lower than those before treatment,and the treatment group was better than the control group(P<0.05);the treatment group was better than the control group in improving the TCM symptoms of flaccid pain,fatigue,abdominal distention,chest tightness,rest and sleep deprivation(P<0.05).Conclusion:The metthods of soothing liver and invigorating spleen and nourishing yin and tranquilizing mind can improve the depressive symptoms and blood sugar level of patients with type 2 diabetes mellitus complicated with depressive disorder.
