Objective: This study explores the clinical efficacy and safety of the Auto Elastic Thread Ligator For Hemorrhoids(RPH-4)plus external hemorrhoidectomy for the treatment of mixed hemorrhoids through a comparative study, aiming to objectively evaluate the advantages and disadvantages of this surgery. Methods: 90 patients with mixed hemorrhoids who met the inclusion criteria were randomly divided into treatment group and control group, with 45 patients in each group.Patients of treatment group were treated with RPH-4 combined with external hemorrhoidectomy, and patients of control group were treated with Milligan-Morgan Hemorrhoidectomy. The total effective rate, operation time, intraoperative blood loss, hospitalization cost,Hospitalization days, wound healing time, postoperative complications and safety were compared between the two groups.Using SPSS19.0 statistical software for analysis.Results:(1)Before treatment, two groups of patients with general information analyzed by the t test and ?2 test,P>0.05,no significant difference between the two groups of data,it was comparable.(2)The total effective rate was 100% in both treatment group and control group, and there was no significant difference between the two groups(P<0.05).(3)The average operation time of the treatment group was 22.40±4.11 min,and the control group was 28.20±4.97 min.The difference between the two groups was statistically significant(P<0.05),and the operation time of the treatment group was shorter.Compared with the control group, the intraoperative blood loss in the treatment group was statistically significant(P<0.05),while the intraoperative blood loss in the treatment group was less.(4)The average hospitalization expenses of the treatment group and the control group were 8534±347 yuan and 7395±466 yuan, the average hospitalization days were 8.04±1.41 days and 10.22±1.68 days. The difference between the two groups was statistically significant(P<0.05),the treatment group had higher hospitalization expenses, but the hospitalization days were shorter.(5)The average wound healing time of the treatment group was 18.11±2.09 days, while that of the control group was 22.57±2.49 days. The difference between the two groups was statistically significant(P<0.05),and the wound healing time of the treatment group was fast.(6)Postoperative hematochezia, anal wound pain and dysuria in the treatment group were significantly different from those in the control group(P<0.05).Postoperative hematochezia, anal wound pain and dysuria in the treatment group were less severe than those in the control group.(7)There was no significant difference in anal bulge and anal edema between the treatment group and the control group(P>0.05).(8)There was no significant difference in the anorectal stenosis and recurrence between the treatment group and the control group(P>0.05).Conclusions: The use of RPH-4 plus external hemorrhoids resection for the treatment of mixed hemorrhoids is effective, safe and reliable, and easy to operate. It can reduce the hospitalization time, shorten the course of treatment, reduce anal cushion injury, significantly reduce postoperative complications such as anal pain, secondary bleeding, and urinary retention, improve postoperative life quality of patients, and truly achieve mixed hemorrhoids surgery minimally invasive and rapid recovery concept.
