Effect Of Phacoemulsification Combined With Capsular Tension Ring Implantation

Objective: In this study,anterior segment OCT was used to observe the effects of capsular tension ring implantation on anterior chamber depth,anterior chamber angle,distance between intraocular lens and posterior capsule,posterior capsular opacification in age-related cataract patients undergoing phacoemulsification combined with intraocular lens implantation.Methods: 34 patients(68 eyes)with age-related cataract aged 52-83 years were collected from May 2017 to October 2017.There were 16 males(32 eyes)and 18females(36 eyes).One eye was randomly selected as the implant group and the tension ring was implanted in the same period of phacoemulsification combined with posterior chamber intraocular lens implantation.The contralateral eye was taken as the control group,and phacoemulsification combined with intraocular lens implantation was performed.Before operation,the two groups were examined by slit lamp microscopy and fundoscopy,including best corrected visual acuity,retina,cornea and crystals.The intraocular pressure of the patients was measured by body and other eyeball structures.Routine B-mode ultrasonography and anterior segment OCT(CASIA SS-1000 OCT,Tomey Corporation)were used to examine the ACD and ACA of the patients.The diopter of IOL corresponding to the patients was calculated according to the results of IOLmaster examination.Best corrected visual acuity(LogMAR)were accurately recorded 2d,3 months and 6 months after operation.The anterior segment OCT was used to measure anterior chamber depth,anterior chamber angle,distance between posterior capsule and lens,and the posterior capsule morphology was observed.Results: 68 eyes of 34 patients successfully completed the operation and follow-up.There was no significant difference in best corrected visual acuity(LogMAR)between the two groups before and 2d,3 months and 6 months after operation(P > 0.05);the control group 0.73±0.37 0.42 ±0.40 0.21 ±0.11 0.21±0.12;the implant group 0.71 ±0.390.37±0.24 0.22±0.10.There was no difference in anterior chamber depth and angle chamber angle(TIA500um)between the two groups 2d after operation(P > 0.05);There were differences in anterior chamber depth and angle(TIA500um)between 3months and 6 months after operation(P < 0.05).The ACD of control group(2.45±0.33)mm(3.77±0.36)mm(3.64±0.28)mm(3.63±0.29)mm,the ACD of implant group(2.45 ± 0.34)mm(3.84 ± 0.32)mm(3.85 ± 0.35)mm(3.82 ± 0.33)mm;The ACA of control group(25.83±7.74)°(43.35±6.55)°(41.87±4.67)°(42.49±4.29)°;The ACA of implant group(27.52±7.43)°(45.40±5.23)°(46.50±3.66)°(45.40 ± 3.70)°.Seventeen eyes in the implantation group had no gap between intraocular lens and posterior capsule,while 8 eyes in the control group had no gap.The distance between the intraocular lens and the posterior capsule was different between the two groups 2d after operation(P < 0.05),The posterior capsular opacification was found in 1 eye of implant group and 6 eyes of control group 6 months after operation.Conclusion:1.Capsular tension ring can reduce the distance between IOL and posterior capsule,prevent the proliferation of lens epithelial cells to posterior capsule and thus reduce the incidence of posterior capsular cataract.2.For age-related cataract patients,capsular tension ring implantation during phacoemulsification combined with intraocular lens implantation can stabilize intraocular lens,deepen the anterior chamber.3.The best corrected visual acuity of the two groups after phacoemulsification and IOL implantation was improved,but there was no significant difference between the two groups.
