Study On Anterior Chamber Morphology Of Myopic Eyes And Its’ Changes After Small Incision Lenticule Extraction

Abstract/Summary:
Purpose:To analyze the normal ranges and distributions of anterior chamber morphologies using Pentacam in Chinese patients with myopic eyes.To investigate the changes of anterior chamber profiles after small incision lenticule extraction(SMILE)and the possible influencing factors.Methods:Section 1:A retrospective study.This study randomly included 338 patients(564 eyes).Pentacam was used to measure anterior chamber depth(ACD),anterior chamber volume(ACV)and anterior chamber angle(ACA)in different orientation.Using Pearson or Spearman correlation analysis to evaluate the correlation between data.Independent sample t-test or Mann-Whitney U test was applied to compare the differences of anterior chamber morphological parameters in different eye and gender.Section 2:A retrospective study.59 eyes of 31 patients with myopia were enrolled and underwent SMILE surgery.The anterior chamber profiles,including ACD,ACV and horizontal ACA were measured using the Pentacam preoperatively and at 1,3 and6 months postoperatively.Pearson correlation was applied to analyze the correlation between parameters.The variance analysis was used to analyze the differences of the parameters at different time points.Results:1.The ACD varied from 2.70 to 3.89 mm with an average of(3.32±0.24)mm.The ACV varied from 145.80 to 302.50 mm~3 with an average of(213.39±28.84)mm~3.2.Male anterior chamber morphology parameters were obviously increased except for temporal and inferior ACA(P<0.05).Anterior chamber morphological parameters were negatively correlated with age excluding superior ACA(P<0.05).Except superior and temporal ACA,the remaining anterior chamber morphology parameters had no significant differences between left and right eye(P>0.05).3.Pupil diameter was correlated with ACV,superior ACA,inferior ACA,nasal ACA and temporal ACA(r=0.124,0.176,-0.180,-0.339,-0.173;P<0.05),not related to ACD(P>0.05).Central corneal thickness(CCT)was negatively related to nasal and temporal ACA(r=-0.123,-0.135;P<0.05),not correlated with other anterior chamber morphology parameters(P>0.05).There was no significant correlation between all anterior chamber morphology parameters and SE(P>0.05).4.ACD was related to ACA in all orientations(P<0.05),ACV positively correlated with nasal ACA.There was a significant difference in ACA between different orientations(P<0.05),temporal ACA>nasal ACA>superior ACA>inferior ACA.5.The PCE decreased after SMILE(P<0.05).There was no significant change in PCE between 3 months and 1 month,6 months and 3 months postoperatively.6.The ACD was significantly decreased after SMILE procedure(P<0.05).There were no significant changes in ACD between 3 months and 1 month,6 months and 3months postoperatively.There was significant difference between ACV preoperatively and 3,6 months postoperatively(P<0.05),but no significant difference was found between ACV preoperatively and 1 month postoperatively(P>0.05).There was no significant change in ACV between 3 months and 1 month,6 months and 3months postoperatively.There was no significant difference in horizontal ACA preoperatively and 1,3,6 months postoperatively(P>0.05).7.There were no significant correlations between△ACD at 1 month postoperatively and age,sex,eye and lenticule thickness(LT)(P>0.05).Conclusions:1.In anterior chamber parameters,the average ACD and ACV were(3.32±0.24)mm and(213.39±28.84)mm~3,respectively,which could provide some references for the diagnosis of clinical diseases and the design of intraocular surgery.2.The normal anterior chamber morphology between the left and right eyes has good symmetry,but the ACA in the same eye is asymmetrically distributed.The morphology of the anterior chamber was not related to the diopter,but correlated with pupil diameter,sex,and age.The ACD,ACV and ACA in males were greater than those in females.With age,ACD,ACV and ACA are all narrowed.The larger the pupil,the smaller the peripheral ACA.Therefore,the clinical assessment of anterior chamber morphology must be integrated multi-factor to consider.3.The PCE,ACD and ACV was decreased postoperatively,and there were no significant changes with time.There were no significant correlations between postoperative△ACD and age,sex,eyes and lenticule thickness.
