Objective :To investigate the risk factors on the formation of inguinal saddle hernia.To provide guidance for preoperative evaluation and intraoperative diagnosis inpatients with inguinal saddle hernia.Reduce the missed diagnosis rate and related complications of inguinal saddle hernia.Methods :The retospective analysis of complete data patients with inguinal hernia were collected from June 2016 to June 2018 in our hospital,this study was based on the Guidelines for the Diagnosis and Treatment of Inguinal Hernia in Adults(2014Edition).Selected and excluded cases according to a same criteria,60 male patients with inguinal saddle hernia were selected for the study and 542 cases of non-inguinal saddle hernia at same time were selected as the control group.Read carefully,screen eligible patients,exclude those who do not meet the requirement.Recording the basic information of patients and the related data of intraoperative observation indicators.The average age of 602 cases is 61.14±17.82 years old.Analysing the age,body weight,body mass index,time of illness,Inguinal hernia size,left and right side,length of inguinal canal and size of herbia ring of these patients who met the enry criteria.SPSS21.0 software was used for statical analysis.T test was used for measurement data and chi-quare test was used for counting data.The risk factors of the formation of inguinal saddle were analyzed by logistic multiple regression analysis.Results :Among the 60 male patients with inguinal saddle hernia,43 cases were included in the medial and region of the inferior epigastric artery.The large hernia sacs had displaced the inferior epigastri artery is 8 cases.There were 9 cases with obvious defect or weakness in the one area and the presence of a herniated mass at other area.The 457 no-inguinal saddle hernia patients with 85 cases indirect inguinal hernia and 8cases with straight inguinal hernia.The age,body weight,body mass index,time of illness,Inguinal hernia size,left and right side,length of inguinal canal and size of herbia ring were analyzed by univariate analysis.The results showed that the body mass index(BMI),time of illness,inguinal hernia size,inguinal canal length and inguinal canal length were associated with the inguinal saddle hernia had significant effect on the occurrence of inguinal saddle hernia(P <0.05),which means that the difference has statistically significant.When these risks were analyzed by multivariable logistic regression,BMI(OR= 1.100,P=0.047),inguinal hernia size(OR=1.756,P=0.000)and time of illness(OR= 1.026,P=0.002)are independent risk factors for inguinal saddle hernia.According to the anatomical characteristics of the inguinal region,the related factors on the formation of inguinal saddle hernia were analyzed.Conclusion :There are many risk factors for the inguinal saddle hernia.The information of inguinal saddle hernia is usually due to the interaction of multiple factors.Preoperative diagnosis of inguinal saddle hernia is diffucult.We found the BMI,inguinal hernia size,time of illnessand,inguinal canal length were independent risk factors for the inguinal saddle hernia.For elderly and obese inguinal hernia patients,intraoperative exploration should be more careful to aviod delaying the developmemnt of inguinal saddle hernia.We should also promote a healthy diet and exercise to avoid obesity.
