Objective: To provide a scientific reference for the choice of clinical operation methods,we compa red the efficacy and complications of two kinds of hernia sac management(open hernia sac and co mplete peeling hernia sac)in laparoscopic indirect inguinal herniorrhaphy(TAPP).Methods: The clinical data of 93 male patients with unilateral type III indirect inguinal hernia wh o underwent TAPP from January 2016 to July 2018 in Xinjiang Production and Construction Corp sHospital were retrospectively analyzed.They were divided into two groups:hernia sac exclusion gro up(62 cases)and peeling group(31 cases).Statistical analysis was made to compare the efficacy and complication rate of the two methods(including operation time and operation area).Bleeding volume,pain VAS score from 6 hours to 24 hours after operation,seroma,scrotal effusion,hernia recurrence a nd other indicators.Results: 1.There were 62 cases in the sputum group and 31 cases in the stripping group,aged 23-75 years,with an average of 56.07±16.50 years old.31 patients in the stripping group;age(56.0±18.1)years old,BMI(23.03±3.08),4 cases of scrotum entering the scrotum,6 cases of type 2 diabetes,2 cases of benign prostatic hyperplasia,62 cases of transverse group,age(56.2 ±15.4)ye ars old,BMI(23.76±2.89),13 cases of scrotum entering the scrotum,1 case of type 2 diabetes,7cases of benign prostatic hyperplasia,no significant difference between the two groups(P>0.05),Both groups of patients were completed by the same treatment group.93 patients in this study suc cessfully completed the operation.The success rate of the two groups was the same,and the diffe rence was not statistically significant(P>0.05).2.Operation time of the sacral group: 67.50(45.00-90.00)minutes,intraoperative blood loss: 5.0(2.0-10.0)ml,24 hours after surgery,VAS score of the operation area: 2(1-3),postoperative The number of hospital stays: 4(3-7)days,were better than the control group,the difference was statistically significant(P <0.05).3.Postoperative complications: There were 8 cases of seroma in the sputum group,the incidence rate was 12.9%,and 1 case of seizure in the strip group,the incidence rate was 3.2%,the difference was not statistically significant(P>0.05).Patients with postoperative seroma in both groups were treated with TDP physical therapy and puncture aspiration therapy.There were 3 cases of scrotal effusion in the sacral group,the incidence rate was 4.8%,and 4 cases in the stripping group,the incidence rate was 12.9%,the difference was not statistically significant(P>0.05).There was a case of hernia recurrence in the sputum group,and there was no recurrence of sputum in the stripping group.The patients with recurrent sputum re-surgery were cured and discharged.According to statistical analysis,there was no statistical difference between the sacral sac and the sacral sac group.Significance(P>0.05).There were no spermatic swelling in the sputum group,and 5 cases occurred in the strip group.The incidence rate was(16.1%),and the difference was statistically significant(P<0.05).Conclusions: Surgical treatment of inguinal hernia with laparoscopic inguinal hernia sacral sac can significantly reduce the complexity of the operation.The time required for surgery,intraoperative bl ood loss,pain from 24 hours after surgery,and postoperative hospital stay The upper part has certai n advantages compared with the complete exfoliation group of the hernia sac.The difference is stat istically significant,and the complication rate is not significantly different from the complete exfolia tion sac.It is worthy of promotion and application in surgical treatment of inguinal hernia.
