Research On The Problems And Countermeasures Of The Combination Of Medical Care And Pension In A Public Hospital In Qiqihar City

Objective:This study takes the combination of medical care and pension old-age care institution established by a public hospital in Qiqihar as an example.Through questionnaires and interviews,it analyzes the problems existing in the combination of medical care and pension in public hospitals,and proposes countermeasures and suggestions.Through the satisfaction survey of the combination of medical care and pension institutions in the public hospitals in Qiqihar City,we will accumulate the practical experience of public hospitals to establish such pension institutions,and establish such pension institutions for other public or private units to improve the quality of their services.reference.Methods:Through literature research,comb the relevant theories and research framework of this research.Taking the combination of medical care and pension old-age care institution of a public hospital in Qiqihar as the research object,a multi-stage random cluster sampling method was adopted to select 143 elderly people from a public hospital in Qiqihar City to conduct a questionnaire survey.Interviews with medical staff,using SPSS20.0 to analyze the survey and interview results,using the ?2 test to analyze the differences in satisfaction of different characteristics of the population,using Logistic regression to analyze the factors affecting the satisfaction of the elderly,and discover the current problems of the organization.And propose countermeasures and recommendations.Results:1.The proportion of elderly people who are satisfied and satisfied with overall satisfaction is 56.64%.The number of people who are satisfied and very satisfied in the six dimensions of living environment,daily care,medical services,spiritual comfort,institutional personnel quality and fee level The proportions are 86.01%,48.95%,41.96%,29.37%,48.25%,and 41.26%,respectively.2.Logistic regression analysis showed that the factors affecting overall satisfaction were academic qualifications and chronic diseases,and the factors affecting daily care satisfaction were age,chronic illness,gender,and factors affecting medical service satisfaction.Education,marriage,occupation,chronic illness,satisfaction factors affecting spiritual comfort,marriage,income,satisfaction factors affecting the quality of institutional personnel,age,education,chronic illness,satisfaction with the level of fees The degree factor is the number of chronic diseases.3.The combination of medical care and pension institution is considered to be in need of improvement in terms of living environment,daily care,medical services,spiritual comfort,personnel quality,and fee level.The most important improvement in all aspects is medical care.Rehabilitation facilities,food tastes,routine physical examination,psychological counseling,and professional literacy of nursing staff accounted for 16.08%,23.78%,20.28%,27.97%,23.78%,and 23.78%,respectively.4.According to the interview analysis,the following problems exist in the development of the old-age institution: the institution's own facilities and services are still not perfect;multi-sector management is difficult to unify and coordinate;the shortage of medical and nursing professionals;the higher the cost level;The consciousness is weak.Conclusion:1.The elderly are not satisfied with the organization's daily care,medical services,spiritual comfort,institutional personnel quality,and fee levels.2.The age,education,income,marriage,occupation,and chronic illness of the elderly have different degrees of influence on the satisfaction of the institution.3.In the management of the old-age care business,the institution has problems of multi-sector management and difficulty in unification and coordination.In terms of personnel training,there is a shortage of professionals in the “medical and nursing combination” and a large loss of personnel.In terms of old-age cognition and communication,there is a lack of awareness of old-age care for the elderly,and there is a lack of effective communication.In terms of old-age expenses,there is a problem that the insurance mechanism is not perfect,the connection between the old-age institutions is not tight,and there is a high level of old-age expenses.4.Suggest the following countermeasures: The government should strengthen supervision and management of old-age care institutions,actively establish a long-term care insurance system,and at the same time strengthen institutional publicity propaganda,expand social awareness,and change people's concept of old-age care;old-age institutions should strengthen their own services.Capacity building,carrying out diversified medical staff training programs,building remote visiting rooms and multi-level pension complexes,continuously improving the level of aged care services and service quality.
