Objective: To find and analyze the influencing factors of intracranial infection in patients after supratentorial craniotomy for tumor resection in the undergraduate department,to find and clear the related influencing factors so as to implement targeted preventive measures and reduce the incidence of intracranial infection after such operations and improve the prognosis of patients with satisfaction.Methods: A retrospective analysis was performed on 2,254 patients who underwent supratentorial craniotomy for tumor resection in our department from June 2016 to December 2018.Finally 1889 patients were selected and included in this study.Based on literature review at domestic and abroad and combined with my clinical experience and previous research results,this study intends to select and analyze the following variables as the related factors affecting intracranial infection in patients after supratentorial craniotomy for tumor resection: gender,tumor type(benign and malignant),tumor site(brain surface,brain parenchyma,ventricle and Anterior-middle cranial fossa),age?60 years,perioperative blood transfusion,emergency surgery,operating time?4h,the operation number?2,preoperative GCS score < 9 points,diabetes,perioperative application hormone therapy,the prophylactic use of antibiotics,perioperative application of EN/PN/ALB,postoperative CT prompt intracranial hemorrhage.SPSS22.0 software was used for statistical analysis,and x squared test and Fisher's exact probability methods were used for comparison of counting data.Multivariate non-conditional logistic regression analysis was performed to analyze the risk factors of intracranial infection after surgery.P<0.05 was considered statistically significant,and the correlation between each factor and intracranial infection after supratentorial craniotomy was analyzed.Results:(1)Of the 1889 patients included in the study,174 developed intracranial infection,with a combined infection rate of 9.2%.(2)Single factor analysis results suggest that tumor type(benign and malignant),tumor site(brain surface,brain parenchyma,ventricle and Anterior-middle cranial fossa),age?60 years,operating time?4h,the operation number?2,diabetes,application of hormone,postoperative CT tip intracranial bleeding are the risk factor for postoperative intracranial infection(p < 0.05),and preoperative prophylactic use of antibiotics can effectively prevent the occurrence of postoperative intracranial infection(p < 0.05).(3)The results of univariate analysis suggested that gender(male,female),preoperative GCS score < 9,perioperative blood transfusion,postoperative indwelling drainage tube,EN/PN/ALB application,and emergency surgery were the non-correlated influencing factors of postoperative intracranial infection.(4)Multivariate logistic regression analysis showed that the factors in(2)were all independent factor affecting the occurrence of intracranial infection.Conclusions: Intracranial infection can be caused by various factors independently or by the joint action of multiple factors including tumor type(benign and malignant),tumor site(brain surface,brain parenchyma,ventricle and Anterior-middle cranial fossa),age?60 years,operating time?4h,the operation number?2,diabetes,application of hormone,postoperative CT tip intracranial bleeding are the risk factor for postoperative intracranial infection,and preoperative prophylactic use of antibiotics can effectively prevent the occurrence of postoperative intracranial infection.Therefore,the perioperative prophylaxis and prophylactic use of antibiotics should be carried out to reduce the postoperative intracranial infection rate.
