Objective:Through questionnaires and in-depth interviews,the awareness of medical disputes among medical staff,patients and health administrators was investigated.Find out the causes of medical disputes and explore effective ways to prevent and solve medical disputes.In order to purify the medical service environment,improve the quality of medical service,build a harmonious doctor-patient relationship and promote the sustainable development of medical and health undertakings,practical and feasible preventive measures and policy recommendations are put forward.Methods: In this study,literature research,expert consultation,questionnaire survey,mathematical statistics,interviews were used to study the current situation and influencing factors of medical disputes in the third-class general hospitals in Nanning city.(1)Expert consultation and literature research were used to familiarize themselves with the relevant information of medical staff and patients.Qu estionnaires and interview outlines were designed and formulated.(2)The questionnaire survey method was used to collect basic data on the relationship between doctors and patients and medical disputes among medicalstaff and patients in the third Class A General Hospital in Nanning.In the pre-investigation,the medical staff of a top three general hospital in Nanning was selected as a pre-survey by a convenient sampling method.60 questionnaires were distributed and 51 valid questionnaires were collected.According to the results of the preliminary survey,the questionnaires were adjusted and revised in time to determine the formal questionnaire.At the time of the formal investigation,the medical staff and outpatient/inpatients of the six top three general hospitals in Nanning were investigated.According to the convenient sampling,562 medical staff questionnaires were distributed,and 559 valid questionnaires were collected.The effective rate of the questionnaire was99.47%.360 patient questionnaires were issued and 357 valid questionnaires were returned.The effective rate of the questionnaire was 99.17%.(3)Based on the mathematical statistics method,the data was analyzed and calculated by spss19.0 software.Descriptive statistical analysis method is used to analyze the basic information of medical staff and patients,to understand the current situation of medical disputes;using chi-square test,from the perspective of medical personnel,it is statistically significant to analyze the incidence of medical disputes from different influencing factors.Multi-factor logistic regression analysis was used to analyze the high risk factors of medical staff medical disputes.(4)By using interview method,three health administrators,six medical staff and six patients were interviewed to understand the awareness of health administrative departments,hospitals and patients about medical disputes and the idea of preventing and reducing medical disputes.Results:(1)Cognitive status: The number of medical disputes in Nanning has increased year by year,and the relationship between doctors and patients isrelatively tight.Among them,the questionnaire showed that there were 182(32.6%)medical staff with very close relationship between doctors and patients.According to interview data,12(80%)of the 15 respondents indicated that the current doctor-patient relationship is tense.(2)Factors influencing medical disputes among medical staff: The results of single factor analysis showed that the gender,education,and job status of medical staff had an impact on the incidence of medical disputes among medical staff,and the results were statistically significant(P<0.05).The results of multivariate logistic regression analysis showed that female medical staff were more likely to have medical disputes than male medical staff in terms of gender,with an OR of 1.566.From the academic point of view,highly educated medical staff are more likely to have medical disputes with lower academic staff,with an OR of 1.712.From the familiarity with risk,the lower the familiarity with medical risks,the greater the possibility of medical disputes,the OR value is1.896.(3)Reasons for medical disputes: Comprehensive medical staff questio nnaires,patient questionnaires and interviews with relevant personnel show ed that patients had high expectations for treatment,high medical expense s,inadequate medical insurance system,unbalanced medical and health res ources allocation,and high-quality medical resources.Too much concentrat ion,poor communication between doctors and patients,lack of trust,and media errors are the causes of medical disputes.Conclusion: According to the research results,the current doctor-patient relationship in Nanning is relatively tight,medical disputes are frequent,and the causes of medical disputes are diversified.The high risk factors for medical disputes among medical staff are women,high education,and low familiaritywith medical risks.Recommendation: First,from the government level,it is necessary to increase investment in medical and health resources,optimize resource allocation,further implement the grading diagnosis and treatment system,and establish a sound legal system related to medical disputes.Second,from the hospital level,it is necessary to strengthen the communication skills communication between doctors and patients,improve the communication skills of medical staff;let medical social workers intervene in clinical doctor-patient communication,ease the tension between doctors and patients;strengthen the hospital's own construction and improve the quality of medical services.Third,from the social level,the public should improve their own health literacy,create a good atmosphere of public opinion,and build a harmonious environment for doctors and patients.
