Objective:To make a dynamic analysis of medical disputes and adverse events in a third-class A hospital in Xinjiang from 2015 to 2018 in order to find out the changing rules and influencing factors and to put forward countermeasures and suggestions for improvement.Methods:1.The adverse events and medical disputes of a third-class A hospital in Urumqi in the past four years(2015-2018)were studied and analyzed dynamically.2.Contrast and analyze the disease and hospitalization characteristics of disputed patients and non-disputed patients,and analyze the influencing factors of medical disputes.Result:1.From 2015 to 2018,3777 cases of adverse events were reported voluntarily,and 239 cases of medical disputes were reported.The incidence of medical disputes and adverse events was the highest in 2017.The correlation analysis of the incidence of medical disputes and adverse events was made.There was a strong correlation between them,P<0.05..2.The structural change of the causes of medical disputes was 45.94%in 2015-2018,and the fluctuation of department structure was 39.96%.During the four-year period,the main causes of medical disputes and key departments have changed greatly;the structural change of adverse events causes was 92.37%in 2015-2018,the highest contribution rate was disputes early warning,the contribution rate was 38.99%,disputes early warning reporting rate has greatly improved.The fluctuation of department structure was 11.6%.3.The single factor analysis of medical disputes showed that the age of patients,the number of days in hospital,the cost of hospitalization,the number of operations,the number of diseases diagnosed in hospital and the results of disease turnover were different,and the proportion of medical disputes was also different.Conclusion:Long hospitalization days,high cost of diagnosis and treatment,the number of diseases diagnosed in hospital and the results of disease turnover are all important factors that may cause medical disputes.We should pay more attention to key factors and patients,identify risk factors in time,and effectively reduce the incidence of medical disputes.There is also a certain correlation between the reporting rate of adverse events and the incidence of medical disputes.Therefore,effective use of the reporting system of adverse events and strengthening the intervention in weak links of medical disputes can reduce medical risks.
