Clinical Study Of Buyang Huanwu Decoction Combined With Shock Wave In The Treatment Of Carpal Tunnel Syndrome Of Blood Stasis Due To Qi Deficiency

Objective:To explore the clinical effect of Buyang Huanwu Decoction combined with shock wave in the treatment of carpal tunnel syndrome of blood stasis due to qi deficienc.Methods:The subjects were all patients who came to the outpatient and inpatient department of Taizhou Bone Trauma Hospital from January 1,2018 to December 31,2018.A total of 150 CTS patients who met the criteria of this study were collected a nd randomly divided into shock wave treatment group(control group 1),Buyang Hua nwu Decoction group(control group 2),Buyang Huanwu decoction combined with sho ck wave treatment group(treatment group),50 cases in each group.On the basis of o ral Mecobalamin tablets,control group 1 was given shock wave therapy,acupoint sele ction:Daling,Neiguan,Shixuan and Laogong;control group 2 was given Buyang Hua nwu Decoction combined with shock wave therapy,and Chinese herbal prescriptions w ere composed of Astragalus membranaceus 60g,Angelica 10g,safflower 6g,Taoren 6g,Chuanqiong 10g,Dilong 10g and Chishao 10g;treatment group:Buyang Huanwu De coction combined with shock wave therapy.Eight weeks later,BMRC functional gradi ng criteria,neuroelectrophysiological indexes,Levine carpal tunnel syndrome questionna ire score and Kelly grade efficacy evaluation were evaluated in the three groups.Results:There was no significant difference in gender,age,course of disease and location of disease among the three groups(P>0.05).During the treatment of three groups of patients,one patient in the control group refused to cooperate because of personal reasons,and was withdrawn from the treatment according to this study;two patients in the control group did not improve after symptomatic treatment of stomach protection because of stomach discomfort after taking traditional Chinese medicine,and one patient in the treatment group was withdrawn from the treatment according to th is study because of working reasons.The results show that:?The evaluation results of BMRC functional grading criteria after treatment in three groups showed that 39 case s in control group 1 had complete remission,7 cases in partial remission and the tota 1 remission rate was 93.88%;38 cases in control group 2 had complete remission,6 cases in partial remission,the total remission rate was 91.67%;48 cases in treatment group had complete remission,1 cases in partial remission,the total remission rate wa s 100%.The total remission rate of the treatment group was the highest.Statistical an alysis showed that there were significant differences between treatment group and cont rol group 1 and control group 2(P<0.05),while there was no significant difference between control group 1 and control group 2(P>0.05),indicating that there was no significant difference in clinical efficacy between the two groups;?The results of co mparison of electromyogram indexes before and after treatment showed that the electro myogram indexes of the three groups were improved after treatment,and the differenc e was statistically significant(P<0.05).There was no significant difference iin the impr ovement of EMG indexes between the two control groups after treatment(P>0.05).Th e improvement of electromyogram index in the treatment group was the most obvious.The electromyogram index in the treatment group was significantly better than that in the control group 1 and the control group 2 after treatment7and the difference was s tatistically significant(P<0.05);?Levine scores of three groups of CTS patients befor e and after treatment showed that the Levine scores of the three groups after treatmen t were improvedcompared with those before treatment,the difference was statistically s ignificant(P<0.05),and the improvement of the treatment group was the most signif icant.There was no significant difference in Levine score between control group l an d control group 2 after treatment(P>0.05).The Levine scores of the treatment grou p were significantly different from those of the other two groups after treatment(P<0.05).?The Kelly grade efficacy evaluation results of three groups of CTS patients af ter treatment showed that the excellent and good rate of the treatment group was 97.96%,significantly higher than that of the other two groups,with statistical significance(P<0.05);the excellent and good rates of the control group 1 and the control group 2 were 85.71%and 81.25%,respectively,with no statistical significance(P>0.05).Conclusions:On the basis of routine treatment with methylcobalamin,this study st udied the therapeutic effect of Buyang Huanwu Decoction combined with shock wave therapy on CTS.At the same time,the clinical efficacy of the two groups was compa red with that of methylcobalamin combined with traditional Chinese medicine.The B MRC functional grading standard,neuroelectrophysiological index,Levine carpal tunnel syndrome questionnaire score and Kelly grade efficacy evaluation of three groups of CTS patients were made respectively.And other research indicators were evaluated.Th e results showed that Buyang Huanwu Decoction combined with shock wave treatment of CTS can significantly improve the functional activity of patients,improve the neur omyogram conduction of patients,and the clinical effect is significant.During the trea tment period,the safety was high.There were no obvious abnormalities in the three g roups through three routine examinations,liver and kidney function tests before and af ter treatment.The results show that the combined treatment of Buyang Huanwu Decoc tion and shock wave can enrich Qi and blood circulation,dissipate blood stasis and d redge collaterals,improve local blood circulation,promote local inflammation absorptio n,relieve local tissue adhesion and relieve pain.The operation is safe and cooperative.The treatment effect is safe and reliable,and is convenient for clinical development.
