Objective: To investigate the status of bone mineral density(BMD)in various parts of type 2 diabetes in Shanghai,to study the relationship between BMD and its various physical indicators,and to analyze the influencing factors of bone mass loss in patients with middle-aged and elderly people.To provide theoretical advice and reference for BMD to promote physical health.Methods: A total of 117 middle-aged and elderly patients with type 2 diabetes mellitus in Yangpu District,Shanghai were selected.The age ranged from 50 to 70 years old.The basic indicators such as gender,age,and duration of disease were recorded and body size indicators(body weight,BMI,body fat percentage)were measured.Physical function indicators(spirometry,maximal oxygen uptake,degree of vascular sclerosis(baPWV)),physical fitness indicators(grip strength,closed eyes,one foot standing,sitting body flexion)and BMD and BMD T values ??of bones in various parts of the body.According to the age of 60,the patients were divided into middle-aged group and elderly group;the disease course was divided into long-term group(>10 years),medium-term group(5-10 years)and short-term group(<5 years)according to the 5-year course;patients were based on BMI Divided into normal weight group(18.5?BMI<24),super-recombination(24?BMI<28)and obese group(BMI?28);patients were divided into baPWV>2400 and baPWV?2400 according to baPWV health criteria;according to grip strength level It was divided into a grip force >18 kg group and a grip strength ?18 kg group;the patient was divided into a normal bone mass group(T>-1)and a bone mass abnormal group(T?-1)according to the BMD T value.The independent sample t test was used to compare the difference of BMD in different parts of each group.Pearson correlation analysis was used to analyze the relationship between BMD and body mass index.The multivariate logistic regression method was used to analyze the influencing factors of bone mass loss in middle-aged and elderly patients with type 2 diabetes.Results:(1)Compared with BMD in all parts of the elderly with type 2 diabetes,male patients had significantly higher BMD than females except for BMD(P<0.01).Female middle-aged patients were in limbs and Total BMD.There was significantly higher BMD than the elderly patients(P<0.05).(2)There were significant differences in BMD between the long-term,mid-term and short-term patients(P<0.01).The difference of BMD was in the short-term to mid-term>long-term;There were differences in the trunk bone,ribs,pelvis,spine bone and Total BMD(P<0.05).The BMD level was in the super-recombination>normal group>obesity group;(3)the patients with high grip strength were lower than the grip strength level.In the limb bones,trunk,ribs and Total BMD,the former was significantly higher than the latter;(4)BMD was significantly negatively correlated with the disease course in middle-aged and elderly patients with type 2 diabetes(P<0.01),and the limb bone and Total BMD and age of female patients There was a negative correlation(P<0.05).There was no correlation between BMD and age in male patients.(5)There was a significant positive correlation between BMD and body weight in middle-aged and elderly patients with type 2 diabetes(P<0.01).Body fat There was a negativecorrelation(P<0.05).Spinal bone and Total BMD were positively correlated with BMI(P<0.05).There was a significant positive correlation between BMD and vital capacity in the body function indicators.There was a significant positive correlation with the grip strength in the physical quality index.(P<0.01);(6)Diabetes duration is a risk factor for bone loss in middle-aged and elderly patients with type 2 diabetes.The 95% confidence interval(95% CI)OR is 1.333(1.156-1.538);It is another factor influencing the patient's bone mass reduction.The 95% confidence interval(95% CI)has an OR of 0.896(0.797 to 1.008).Conclusions:(1)BMD in males with type 2 diabetes is better than that in females.BMD in female patients decreases with age,while age changes have little effect on BMD in male patients;(2)BMD in middle-aged and elderly patients with type 2 diabetes Affected by the course of disease and obesity,the longer the course of disease,the lower the BMD,the appropriate increase in body weight and the maintenance of body type can enhance the BMD of patients;(3)The BMD of middle-aged and elderly type 2 diabetic patients is related to muscle strength and lung function,and the muscle strength is larger.The stronger the function,the more obvious the effect of enhancing BMD in patients;(4)Strengthening physical exercise,especially increasing resistance exercise,and the proper amount of aerobic exercise is important for enhancing BMD and preventing bone loss in middle-aged and elderly patients with type 2 diabetes.effect.
