Abstract/Summary:
Objective: The bone density of the middle-aged and elderly in 13 communities in Shihezi was measured and the database of bone density was established,the influence factor of bone mineral density(BMD)status was analyzed and discussed,which may provided the preventive measeurs and treatment of osteoporosis.Methods: The subjects were collected from 13 comunities(include 20,21-1,21-2,34,42,48,50,63,78 and Long Yan comunity),interviewed face-to-face with questionnaires was taken,the clinical indicators include gender,age,height,weight,waist circumference,hip circumference,blood pressure,complications lifestyle and eating habits were recorded.Body mass index(BMI)was calculated by kg/m2。2.Bual energy X-ray absorptiometry(DEXA)was used to measure the BMD of L1-L4,total Lumbar,femoral neck,Wald triangle,data was showed with the unit of g/cm2.The subjects were diveded into normal controls,osteopenia and osteoporosis group.3.The glucose metabolism indicators such as fasting blood glucose(FBG),Glycosylated hemoglobin(Hb A1c),The bone metabolism indicators such as Calcium(Ca),Phosphorus(P),Alkaline phosphatase(ALP)and lipid metabolism indicators such as Cholesterol(Ch),High density lipoprotein(HDL),Low density lipoprotein(LDL);renal function indicatos such as Creatinin(Cr),Blood urea nitrogen(BUN),Uric acid(UA);hepatic functional indicators such as Alanine transaminase(ALT),Aspertate aminotransferase(AST)were measured and collected,the differences between different groups were compared,and the correlation of BMD and indicatos were analyzed and the influence factors of BMD were analyzed.4 SPSS 19 was used to analyse the data,the measurement data with normal distribution showed with mean standard deviation(X ± S),variance analysis was used in groups.Pearson correlation analysis was used to the correlation of indicators.the difference between the level of data using the chi square test,logistic regression was applied to analyze the factors of BMD the influence of P < 0.05,there were statistically significant differences.The chi square test was used to analyze ranked data.Logistic regression was used to analyse the influence factors of BMD,the difference was statistically significant with P<0.05。Results: 1.Atotal of 858 subjects were enrolled in this study.The age ranged from 46 to 90(68.42±9.35),with a total of 331 males and 527 females.The detection rate of OP was 23.6%.2.The normal bone mass group age(60.54± 11.81),osteopenia group age(65.85 ± 7.82)and OP group age(71.15±6.84),compared with the three groups,the age was increased gradually,p <0.05.The normal bone group BMI(26.66 ± 3.21),osteopenia group BMI(24.68± 3.41),OP group BMI(22.69 ± 3.47),when the three groups were compared,the BMI was decreased gradually,P <0.05.Compared OP group Cr(77.98±23.15),BUN(6.28±5.73),P(1.18±1.07),Ca(2.26±0.37),ALP(75.21±26.19)with normal bone group Cr(.83±25.57),BUN(49±2.02),P(0.96±0.22),Ca(2.18±0.37),ALP(66.27±15.42),their values were lower than those of normal bone mass,the result showed P <0.05.Compared OP group UA(286.57±79.74)with normal bone group UA(319.61±95.90),their value was lower than normal bone mass,the result showed p<0.05.3.There was a positively correlation between BMI,UA and BMD(r=0.302,0.155,p<0.05).There was a negatively correlation between Age,Cr,BUN,P,ALP and BMD(r=-0.095,-0.262,-0.099,-0.060,-0.147,p<0.05).4.Among The normal bone mass,osteopenia group and OP group,the proportion of female menopause were 60.4%,98.8%,100%,chi square value was X2=36.415,p<0.01.5.Logistic regression analysis showed whether eated calcium,sex the difference in bone mineral density influencing factors,and the OR values were 2.587,6.937,p<0.05.6.ROC curve detection areas of different BMD in the sites of L1,L2,L3,L4,Ltotal,femoral neck and Ward triangular were 0.906,0.917,0.922,0.894,0.929,0.883,0.903,P<0.01.Conclusion: 1.The detection rate of OP in middle-aged and elderly people in Shihezi community was 23.6%.Compared with the average level of our country,the situation is not optimistic.2.BMD was correlated with biochemical indexes of BMI,P,ALP,Cr,BUN,UA and age,sex,female menopause.3.Calcium supplements could reduce the occurrence of OP as soon as possible in middle-aged and elderly people in shihezi area.4.It is more diagnostic significance that average total lumbar spine BMD diagnose OP in clinical.
