Study On The Influencing Factors Of Clinical Prognosis In Elderly Male Patients With Alzheimer's Disease

Objective:Alzheimer's disease is the most common clinical type of dementia,many studies have shown that the risk factors and disease progression of AD disease,but less related to the factors affecting the prognosis of the disease,through the analysis on the prognosis of Alzheimer's disease,to investigate the factors influencing the elderly Alzheimer's disease patients.Method:(1)In the research group collected 60 men over the age of 96 patients with AD,all of the subjects completed the questionnaire and clinical data of the health status of the nervous system of unified self collection,in line with the United States(DSM-IV)in the diagnosis of AD,the national standard of psychiatry and communicative disorders and stroke and Alzheimer's disease and related research the disease association(NINCDS-ADRDA)diagnostic criteria may be set by the AD.After 3 years of hospitalization,outpatient and telephone,96 aged male AD patients were divided into the death group and the survival group on the basis of 3 years'survival.(2)The demographic and clinical data of all aged AD patients were collected,and the database was established by Epidata3.1 software,and the SPSS22.0 sof tware was analyzed statistically.Measurement data using the mean and standard deviation(`x±s)said that the groups were compared using independent sa mples t-test;Count data with the number or percentage of cases,groups were compared by Chi-square test;Survival analysis was performed on elderly male AD patients using life table method to calculate the 3-year survival rate of eld-erly male AD patients.using Cox regression model to analyze the effects of risk factors on the prognosis of Alzheimer's disease in elderly men.P<0.05indicates that the difference is statistically significant.Results:(1)In this study,96 elderly male AD patients were investigated.After 3 years of follow-up study,42 elderly patients who died of AD accounted for 43.75%of the total number of AD patients.(2)After chi-square test,the two groups of patients achieved the following results:Patients with bedridden bed had higher AD mortality than those without bedridden(X~2=9.407,P<0.05);patients who did not exercise were more likely to die than those who did exercise.The rate was high(X~2=10.060,P<0.05);patients with mental depression had a higher AD mortality rate than those with no depression(X~2=7.934,P<0.05).Therefore,there was a significant difference in the general demographic data between the survival group and the death group in bed rest,physical exercise and mental depression(P<0.05);(3)The following results were obtained after t-test in both groups:Survival patients had higher diastolic blood pressure than patients who died(t=0.728,P<0.05).Patients in the survival group had lower C-reactive protein levels than patients in the death group(t=-2.162,P<0.05),Patients in the survival group had lower levels of homocysteine than those in the death group(t=-4.337,P<0.05).Patients in the survival group had lower creatinine levels than those in the death group(t=-3.165,P<0.05),Patients in survival group had lower urea levels than those in death group(t=-3.469,P<0.05),patients in survival group had lower fasting blood glucose levels than those in death group(t=-3.404,P<0.05),survival group patients The high-density lipoprotein levels were higher than those in the death group(t=2.572,P<0.05).Therefore,the clinical data of survival group and death group were statistically different in diastolic blood pressure,C-reactive protein,homocysteine,creatinine,urea,fasting blood glucose and high-density lipoprotein(P<0.05).(4)The single factor analysis with statistical difference factors included in the multivariate Cox regression analysis,the results showed that homocysteine(RR=5.028,95%CI=1.219-9.536),fasting blood glucose(RR=4.351,95%CI=1.328-10.512)is a risk factor for prognosis of elderly male patients with AD.Conclusion:(1)The prognosis of elderly patients with AD is not good and the mortality rate is high.(2)Patients with AD who are bedridden,lack physical exercise,and suffer from mental depression have a worse clinical prognosis.(3)The poor control of diastolic blood pressure,C-reactive protein,creatinine,homocysteine,urea,fasting blood glucose,and high-density lipoprotein has a certain influence on the prognosis of AD and increases the risk of death.(4)Plasma homocysteine may become a serum marker for Alzheimer's disease Plasma homocysteine and fasting blood glucose provide new screening, treatment,and follow-up for high-risk groups of elderly Alzheimer's disease.
