An Analysis Of Perception And Impacting Factors Of Patient Safety Culture Among Medical Staffs In A Third Grade Class A Hospital In Shenyang City

Objective:This article investigates the current status of medical staff' cognitive to patient safety culture in a third grade class A hospital in Shenyang city,through the questionnaire survey.And compare the different characteristics of the medical staff in patient safety culture's cognitive level,analysis the influence factors of the patient safety culture cognition,and clearly the advantage and disadvantage field of hospital safety culture.in order to provide suggestions to improve the hospital patient safety culture and improve the level of hospital management.Methods:The use of literature reaearch to systematically search for relevant literature on patient safety culture provides a theoretical basis and methodolodical reference for this study.By means of questionnaire survey,medical staff of a third grade class A hospital in Shenyang city are taken as the research object,and stratified random sampling was adopted.A total of 500 questionnaires were distributed,and 464 effective questionnaires were collected,with an effective rate of 92.8%.Taking the HSOPSC as the survey tool,the survey contents the basic information of medical staff and the cognition of patient safety culture.In this study,we use Epidata3.0 to double the data and establish the database,and use SPSS 21.0 for data processing and analysis.Descriptive statistics were used for basic information of respondents,Independent t test and variance analysis were carried out for cognition on patient safety culture of medical staff with different characteristics.Correlation analysis was used to analyze the relationships among the dimensions of cognition on patient safety culture,and multiple linear regression analysis was used to analyze the important factors influencing the safety culture of medical staff,and P<0.05 was for the difference was statistically significant.Results:1.The 464 medical staff surveyed had a score of(3.68±0.94).Among all dimensions of perception,the score of team cooperation was the highest(4.34±0.63).The score of dimension on Organize learning and continuous improvement,feedback and communication on errors,and manager's willingness and actions to promote safety was higher,with the lowest staffing score(2.40± 0.84).2.The positive response rate of overall patient safety culture was 57.85%,In terms of the positive response rate of each dimension,the team cooperation in the department,the willingness and action of managers to promote safety,the positive response rate of organizational learning and continuous improvement are higher than 75%,which is the advantage dimension.Incident reporting frequency,personnel allocation,and positive response rate to errors are less than 50%,belonging to the dimension which to be improved.Among all items,the lowest positive response rate was 8.6%,it is belong to the A5 item“ undergraduate employees who worked longer than the time of patient safety care”.The positive response rate of all three items in the dimension of incident report frequency was less than 50%.3.Through multiple linear regression analysis,the section is the cognitive factors of patient safety culture,different section's score has difference which was statistically significant in the total score,organizational learning and continuous improvement,and hospital management support,the overall feeling of patient safety culture,communication openness,succession and change one's major,response to the error of the punishment and collaboration between departments(F=2.793,3.592,3.065,4.007,2.602,4.007,2.602,P< 0.05).The patient safety score of the department of obstetrics and gynecology was the highest(168.67±20.43).Scores were lower in internal medicine and surgical(154.98±17.61)(153.99±18.09).Form of employment is the cognitive factors of patient safety culture,It showed statistically significant differences in the openness of communication,personnel allocation,non-punitive response to errors and inter-departmental collaboration dimensions(F=4.204,5.330,3.296,3.415,P<0.05),The patient safety score of the contract employees was higher than that of the official employees and personnel dispatchers.The times of knowing the safety of patients is the influencing factor of cognition on safety culture of patients.The more the number of times,the higher the total score of cognition of patient safety culture.Teaching or not is the influencing factor of safety culture cognition of patients,The differences were statistically significant on the willingness and actions of managers to promote safety?transfer and interdepartmental collaboration(t=-2.159,-2.049,-2.011,P<0.05)? The score of untrained medical staff was higher on these three dimensions.The familiarity degree of patient safety culture is the influencing factor of patient safety culture cognition.In addition to the non-punishment response dimension of errors,all other dimensions and the total score show statistically significant differences(P<0.05).Conclusion:1.The overall perception score of patients' safety culture by medical staff in the hospital investigated by the research institute was(3.68±0.94).Among all dimensions of perception,the team cooperation score in the department is the highest(4.34±0.63),and the score in the dimensions of organizational learning and continuous improvement,hospitals and actions promoted by managers for safety,and feedback and communication on errors is relatively high,and the score in personnel allocation is the lowest.2.Team work within the department and management to promote safe hospital and organizational learning and continuous improvement are the advantages of the patient safety culture of the hospital,incident reporting frequency,staffing and non-punitive response to errors are the areas to be improved in the patient safety culture of the hospital.3.Medical staff in the department,the form of employment,the number of understand the patient safety,whether to teach and familiarity with patient safety culture is the influencing factor of safety culture cognition of patients in the hospital.
