| Objective: to observe the clinical effect of wen tong du mai and oral western medicine in treating rheumatoid arthritis of cold dampness.To clarify the effect of wen tong du mai method on the treatment of cold dampness bi type rheumatoid arthritis,to further explore its mechanism,to provide a new effective treatment for this disease.Methods: according to the inclusion criteria of this project,72 cases of rheumatoid arthritis patients with cold and damp arthralgia in the department of acupuncture and rheumatology,affiliated hospital of changchun university of traditional Chinese medicine were included,and the patients were divided into treatment group(36 cases)and control group(36 cases)according to the random grouping method.In the treatment group,moxibustion was performed at the major vertebus,body column,lingtai,zhiyang,tendon contraction,central center,mid-spine,hanging pivot,mingmen,waiyangguan and waishu at the major vertebus acupoints,once a day for 30 min each time.According to the patient's physical condition,about 0.8-1 inch needle was injected,and the needle sensation was appropriate to reach the sense of acid,numbness and swelling pain.The needle was kept for30 min once a day.In the control group,the oral dose of methotrexate tablets was increased by 2.5mg per week starting from 10 mg,and maintained at 10mg-15 mg per week once per week according to the tolerance level.Both groups received 1 course of treatment on 7 days and received 4 courses of treatment in total.The changes of TCM syndrome scores,clinical symptoms and signs(joint tenderness number,joint swelling number,VAS,morning stiffness time)and physical and chemical examination(ESR,CRP)before and after treatment were observed to compare the efficacy of the two groups.Results: after statistical analysis,P > 0.05 was comparable between the two groups in age,gender,course of disease,joint function grade,clinical symptoms and signs,and TCM syndrome score.In terms of clinical symptoms and signs,there were differences in the number of joint swelling,tenderness,pain degree and morning stiffness between the two groups beforetreatment.Statistical analysis showed that P > 0.05 had no statistical significance,and the two groups were comparable.After treatment,there were differences in the number of joint swelling,tenderness,pain degree and morning stiffness between the two groups.After statistical analysis,P < 0.05 was statistically significant.The differences of TCM syndromes before treatment between the two groups were not statistically significant and comparable.After treatment,the difference of TCM syndrome scores between the two groups was statistically significant(P < 0.05).The effective rate was96.88% in 31 cases in the treatment group.The total effective rate was 84.38% in the control group.However,after statistical analysis,P>0.05 had no statistical significance.Comparison of erythrocyte sedimentation rate and c-reactive protein before treatment between the two groups showed no statistical significance after statistical analysis(P > 0.05),suggesting that the two groups were comparable.After treatment,the comparison of erythrocyte sedimentation rate between the two groups was statistically significant(P <0.05).Comparison of c-reactive protein after treatment between the two groups showed no statistical significance(P > 0.05).Conclusion:1.The method of warming,tongdu,and oral administration of western medicine have a good therapeutic effect on rheumatoid arthritis of cold dampness bi type.2.The effect of warming,tongdu pulse treatment on the number of joint swelling,number of joint tenderness,degree of pain,morning stiffness and other clinical symptoms in patients was better than that in the control group.3.Wen tong and du pulse method was statistically superior to the western medicine group in TCM syndrome score.4.Wen tong du pulse method is safe and effective,non-toxic side effects,is worth using in the majority of clinical. |
