Objective:To compare the effect of fire needle therapy and diclofenac sodium based on the conventional antirheumatic drugs,observe the fire needle treatment of cold-dampness type rheumatoid arthritis(RA)clinical efficacy,at the same time,test the efficacy and safety of acupuncture in the treatment of rheumatoid arthritis.Methods:The cold-dampness type of rheumatoid arthritis patients from December,2016 to January,2018 in acupuncture department of rheumatology of Nanjing Hospital of Chinese Medicine were divided into fire needle group and control group based on the randomized table.Fire needle group oral methotrexate tablets 10mg weekly,leflunomide tablets 10mg,once a day;in the meantime given the fire needle treatment,With the patient prone,fully exposed parts of operation,after strict disinfection of acupoints burn the milli fire needle on the alcohol lamp burning from red to white,quickly after piercing Jiaji and local acupoints,Immediately the needle is out and then the pinhole is pressed with a sterilizing dry cotton ball.twice a week,treating 4 weeks as a course,two courses were observed.Patients in the control group were given methotrexate tablets 10mg once a week,leflunomide Tablets 10mg,once a day,anti-inflammatory and analgesic drugs:diclofenac sodium sustained release tablets 75mg once a day.4 weeks as a course of treatment,a total of 2 courses of treatment.ESR?RF and CRP were measured in two groups before and after the treatment,the visual analogue score and TCM syndrome scale were filled in.Results:(1)Before the treatment,the two groups were compared(P>0.05)with comparability.(2)Comparation of the serological index:ESR,RF and CRP were compared within the two groups before and after the treatment by paired t test,both P<0.05,the difference was statistically significant.The level of ESR(P=0.01),CRP(P=0.01)and RF(P=0.03)between the two groups,which means significant difference.(3)Visual analogy score:In the two groups before and after treatment by paired t test P<0.05,the difference was statistically significant,comparison between the two groups after treatment:the fire needle group were decreased more obvious between the two groups have significant difference.(4)TCM syndrome scale:The scores of the symptoms of the two groups were compared before and after the treatment.The difference was superior to the rank sum test of P<0.05.Before the treatment of the two groups,the symptom score was compared with P>0.05,and the difference was not statistically significant,and the difference was comparable.After treatment,there were significant differences in joint pain with cold(P=0.008),joint swelling(P=0.001),joint stiffness(P=0.038),aversion to wind or cold(P=0.047)and wiry and tense pulse(P=0.037)between the two groups.(5)In the total efficiency:after two courses of treatment,the total efficiency of acupuncture group was 90%,control group total effective rate was 60%,the difference was statistically significant,The total efficiency of fire needle group was obviously higher than the one of control group.(6)In the aspect of safety:fire needle group had no adverse reaction in patients with syncope,local infection,patients in the control group there were three cases with abdominal distension and other gastrointestinal adverse reactions,The safety of acupuncture group was better than the control group.Conclusion:Fire needle group is better than the control group in the improvement of laboratory indicators such as ESR,CRP,ESR,VAS and the symptoms of joint pain with cold,joint swelling,joint stiffness,aversion to wind or cold with wiry and tense pulse.
