Object:1.To observe the effect of lemon and ginger juice on CINV in breast cancer patients:2.To compare the differences in the effect of oral, gargle and aromatherapy inhalation three ways of lemon and ginger juice on the chemotherapy-related nausea and vomiting in patients with breast cancer,in order to find out a better curative effects and lower side effects intervention method for clinical prevention and treatment of chemotherapy-related nausea and vomiting. Methods:This was a clinical randomized controlled study.Cases selected for the period from October 2016 to August 2017 the Guangdong Provincial Hospital of Traditional Chinese Medicine Hospital in patients with meeting the inclusion criteria of 112 cases with breast cancer, and then they were randomly divided into four groups:the control group(n=28),the Oral group(n=28),the gargle group(n=28),the aromatherapy inhalation group(n=28).the control group was applied with conventional treatment and nursing.Based on the controlled group,At half an hour after chemotherapy, the oral group was applied with oral administration of fresh lemon ginger juice(10ml lemon juice and 6ml ginger juice)into 300ml of warm water and instructed patients to take orally,30ml each time, interval of half an hour, two days totally.Based on the control group, the same lemon ginger juice was squeezed into warm 300ml boiled water and rinsed with each patient for the time of 3-5 minutes,30ml each time, interval 0.5h,two days totally in the lemon ginger gargle group.based on the control group,at half an hour after chemotherapy, added the same dose of fresh lemon ginger juice into300ml of warm water and then into the aromatherapy machine, and instructed patients to inhale aromatherapy, about 79ml each time, about 20min every time, four times a day in the aromatherapy inhalation group. Before the experiment, the general data and the KPS of the patients were collected. The severity of nausea, vomiting and retching symptoms with patients were evaluated with the INVR at 24h before chemotherapy,12h after chemotherapy,24h after chemotherapy,24h after chemotherapy. At the same time, the appetite, the change of mood, fatigue and other associated symptoms related with chemotherapy were observed and compared.Throughout the experiment used the adverse reactions observed table to record the adverse reactions. The data were entered into SPSS20.0 for statistical analysis and treatment.Results:1.Mitigation effect of lemon ginger on CINV in breast cancer patients(1)The comparsion between controlled group and intervened groups:At D12h after chemotherapy,There were statistically significant difference in overall symptoms, nausea symptoms, and retching symptoms between the group B,group D,and group A.At D24h and D48h after chemotherapy, there were significant differences between group B and group A,group D and group A in the overall symptom score of CINV,nausea, retching; The vomiting symptoms were statistically significant at D12h,D24h after chemotherapy, but not statistically significant at D48h(P>0.05);Compared with group A,the scores of the overall symptoms, nausea symptoms and retching symptoms at chemotherapy D12h were statistically significant(P<0.05),and the scores of retching symptoms of chemotherapy D24h were statistically significant(P<0.05)There were a statistically significant difference in the overall symptoms and nausea scores of chemotherapy at D48h(P<0.05);(2)Comparsion among three intervened groups:At the D12h of chemotherapy, the scores of the overall symptoms, nausea, and retching symptoms among group B,group D,and group A were significantly different(P<0.01).At D24h and D48h,there were no significant differences among group B,group C and group D in the overall symptom score of CINV,nausea and vomiting; The scores of retching symptoms between group C and D at D12h,group B and group C at D48h were statistically significant(P<0.05).2.The symptoms of chemotherapy correlation associated(1)The comparsion between controlled group and intervened groups:There was statistically significant difference in the severity of depression in group D and group A(P<0.01),and there were no significant difference between group C,group B and group A(P>0.05).,and there were no significant differences in appetite, fatigue, sleep condition(P>0.05);there were significant differences in incidence of dizziness and headache among four groups(P<0.05);Although the incidence of constipation was not statistically significant, the difference was obvious(P=0.05);there were no significant differences in the incidence of Bloating and diarrhea(P>0.05);(2)Comparsion among three intervened groups:patients with depression severity group D and group C,group D and group B differences were statistically significant(P<0.05);The incidence of constipation was group B<group D<group C,there were no significant difference among group B,group D,group C;the incidence of dizziness and headache were group D<group B<group C;although the comparsion between group D and group C in dizziness incidence was not statistically significant, but significant difference(P =0.06);there were no significant difference among other groups(P>0.05);3.dverse reactionsDuring the experiment,a total of 4 cases of adverse reactions, accounting for 3.84%of 110 cases, three was each one patient had transient blush fever and the temperature was normal by mercury thermometer in group A and groupia D;three was each one patient with minor night sweats symptoms;after the discussion of medical experts believe that the four cases of adverse reactions are caused by the use of chemotherapy drugs and drug-induced metabolism and had nothing to do with the use of lemon ginger.Conclusion:1.Lemon ginger can effectively alleviate the symptoms of CINV in breast cancer patients, especially in the nausea, retching and acute vomiting symptoms significantly;2.The lemon ginger aromatherapy inhalation method has a very significant improvement effect on the degree of depression of patients, and has a certain degree of relieving effects on dizziness and headache symptoms;lemon ginger oral method can relieve constipation in patients to some extent;3.From the perspective of overall curative effect, the methods of aromatherapy inhalation and oral administration were effective in alleviating the symptoms of CINV and were all superior to the gargle method;4.There was low adverse reaction rate when use the lemon ginger to prevent and treat CINV,it is no side effects, safe.
