Part One The cognitive status of the ICU nurses in ABCDE bundleObjective To investigate the cognition status of ICU nurses on analgesia and sedation cluster strategy in the 3A Grade Hospitals of Henan Province,and to analyze the general demographic related factors.Methods On the basis of literature review and expert consultation,a general information questionnaire and an analgesic sedation cluster strategy cognitive questionnaire were designed.A cross-sectional survey was conducted among 280 ICU clinical nurses who met the inclusion and exclusion criteria.SPSS17.0 statistical software was used to establish the database and statistically analyzed.Statistical methods such as descriptive statistics,single factor analysis of variance,t test and multiple linear regression analysis were used.The test level is α = 0.05.Results A total of 280 questionnaires were sent out,and 243 valid questionnaires were returned.The cognitive score of ABCDE Bundle strategy was 81.65 ± 7.26.The average score of basic knowledge items was 2.17 ± 0.25,and the average score of implementation method was 1.99 ± 0.41,and the average score of comprehensive application ability items was 1.86 ± 0.18.The results of single factor analysis showed that ICU nurses’ perceptions of analgesia and sedation cluster strategy were related to three factors,including educational background,ICU working years and specialist training(P < 0.01 or P<0.05).Multiple linear regression analysis showed that educational background,ICU working years and specialist training were the influencing factors of ICU nurses’ perceptions of ABCDE Bundle strategy.Conclusion 1.ICU nurses have poor overall cognitive level of ABCDE Bundle strategy.2.ICU nurses’ perceptions of ABCDE Bundle strategy were influenced by their general demographic factors.Among them,the ICU working years,specialist training and academic degree were the most significant impacts.3.ICU nurses need to be trained the systematic knowledge of ABCDE bundle.Part Two The cognitive intervention study of the ICU nurses’ ABCDE bundleObjective To explore the influence of systematic knowledge training on nurses’ cognition of analgesia and sedation cluster strategy,so as to provide reference for ICU managers to implement cognitive intervention.Methods This stage was designed for exprimental research.Based on the cross-sectional survey,two ICU deprtments with similar scale and environment were randomly divided into control group and exprimental group and the nurses in intervention group 42 cases)and control group(32cases)were selected by convenience sampling method.The study lasted 5 weeks,the control group participates in established routine business learning;on the basis of informed consent,the intervention group adopted collective teaching,network knowledge push and bedside teaching methods to train the knowledge of analgesia,sedation and cluster strategy system once a week.The general information questionnaire and the analgesic sedation cluster strategy cognitive questionnaire were used before and after intervention,and the intervention effect was evaluated.The collected data were statistically analyzed by using SPSS17.0 statistical software,and descriptive statistics,t test,rank sum test and other statistical methods were used.Inspection level alpha =0.05.Results Two independent samples t test results showed that there was no significant difference between the two groups in general demographic data,total score of analgesia and sedation cluster strategy and the scores of each dimension before intervention(P > 0.05),and the results were comparable.The total score of analgesia and sedation cluster strategy of the intervention group was higher than that of the control group,the difference was statistically significant after intervention(P<0.01).The scores of each dimension in the two groups were statistically significant(P<0.01).Conclusion The system knowledge training can effectively improve the cognitive level of ICU nurses’ ABCDE Bundle strategy.
