Objective:To observe the expression changes of Periostin(PN or OSF-2)in the tissue and serum of normal oral mucosa,oral leukoplakia and oral squamous cell carcinoma,to analyze its significance in the development of oral squamous cell carcinoma.For the detection a reference index to monitor the occurrence,development and prognosis of oral precancerous leukoplakia and squamous cell carcinoma.Methods:Part I:(1)Outpatients and inpatients,divided into 30 cases of normal oral mucosa,40 cases of oral leukoplakia and 45 cases of oral squamous cell carcinoma,periostin protein was tested by immunohistochemical staining(IHC)of these parafin sections;(2)Quantitative real-time polymerase chain reaction(q RT-PCR)was used to observe the expression of periostin m RNA in the fresh tissues of in the three groups which included 30 cases of normal oral mucosa,25 cases of oral leukoplakia and 45 cases of oral squamous cell carcinoma.Part II:(1)The serum periostin concentration in the three groups which contained 30 cases of healthy subjects,25 cases of oral leukoplakia and 45 cases of oral squamous cell carcinoma,was detected by enzyme linked immunosorbent assay(ELISA);(2)Periostin expression in patients with oral squamous cell carcinoma was analyzed by age,gender,degree of differentiation,lymph node metastasis,TNM stage and depth of invasion.Results:Part I:(1)Immunohistochemical staining results showed us that in normal oral mucosa,hyperplasis leukoplakia,dysplasia leukoplakia and oral squamous cell carcinoma tissue area index positive cells and integrated optical density were(2.51±1.30,15.41±1.63),(2.70±1.26,15.66±1.17),(14.23±0.42,33.13±1.07),(33.47±1.77,51.29±2.67).There were statistical difference of the index of positive cell vessels and the value of IOD of Periostin between every two groups except for oral normal mucosa and hyperplasia leukoplakia(P<0.05).And in oral squamous cell carcinoma,the expression quantity of periostin depend on the patients' tumor differentiation degree,also TNM staging and lymph node metastasis were taken into account(P <0.05);(2)Periostin m RNA levels in all three groups have expressed,Periostin m RNA in normal oral mucosa,oral leukoplakia and oral squamous cell carcinoma tissue were 0.326074±0.412194,0.751156±0.250896,1.377044±0.500052.From the perspective of gene levels of periostin we found that the expression of periostin ascended as the degree of malignancy increases,and the difference of expression was statistically significant(P<0.05);(3)The two test ways that fluorescent quantitative PCR and immunohistochemistry showed good agreement in the results of periostin tes(Kappa=0.932>0.81).Part II:(1)In healthy subjects and OLK the serum of Periostin were 54.23±3.8 ng/ml and 20.64±5.3 ng/ml.The serum concentration of periostin was 81.06 ±10.1 ng /ml in OSCC which was significantly higher than those in OLK or healthy subjects(P<0.05);(2)the serum expression quantity of periostin depend on the patients' tumor invasion,also TNM staging and lymph node metastasis were taken into account(P <0.05).Conclusions:(1)From the view of m RNA and protein levels,the expression intensity of periostin gradually strengthen as the degree of malignancy increases,which can be provied that the high expression of periostin closely related to the occurrence of OSCC.(2)The immunohistochemistry and quantitative real-time PCR method showed good agreement in the results of periostin test,so that immunohistochemistry could be checked before gene detection due to its less simple operation and attractive price.(3)Periostin expression in patients' serum could be act as a potential director to monitor the development of OSCC,simultaneously it also could be serviced for the judgment basis of tumor invasion and prognosis.
