Objective1.To understand the epidemiological characteristics of varicella in Hedong District of Tianjin from 2010 to 2013.2.To analyze the characteristics of varicella event,clinical features of varicella cases involved in the event.To explore the related factors of varicella event.3.To analyze economic burden of patients and expenditure of disposal in the varicella event,and analyze cost-effectiveness providing scientific evidence for developing prevention and control measures of varicella.Methods1.Relavant information of varicella incidence in Hedong district of Tianjin from 2010to 2013 was from Notifiable Infectious Diseases Reporting Information System.Data of varicella event(case aggregation and outbreak)from 2010 to 2013 were collected by infectious disease control department in Hedong District CDC of Tianjin.The economic burden of cases in outbreak,the cost of disposing the outbreak,the economic loss expected and cost-effectiveness were analyzed.2.All the analyses were carried out by chi-square test,trend chi-square test using SPSS 16.0 software.The possible factors impacting epidemic duration of varicella event were analyzed by multiple linear regression model.Results1.2684 cases of varicella were reported in Hedong District from 2010 to 2013,and1446 men and 1238 women among them.The average annual morbidity was74.22/100000 and decreased year by year.The incidence rates in men and in women were 78.37/100000 and 74.08/100000,respectively.There was no significant difference between men and women(X~2=2.172,P>0.05).Incidence rate increased with age from one year old and reached the highest incidence(642.50/100000)in the group of 10 to 15 years old,then decreased gradually.The varicella often occurred in spring and winter(from March to May and November to next January),and more serious in winter than in spring.Varicella cases were reported in all 12 streets.The top three places of annual incidence rate were Dazhigu Street(98.88/100000),Xiangyanglou Street(97.17/100000)and Erhaoqiao Street(92.81/100000).2.28 varicella events were reported in Hedong District of Tianjin from 2010 to 2013,8 events were up to outbreak among them.Most events occurred in primery school(23 events,82.15%).The events had two peaks in spring and winter(from March to April,from November to December),respectively.The varicella event and outbreak were high in November.Most events occurred in schools and four schools had more than two outbreaks.The case ratio of male to female were 1.24:1 and most occurred between 7 and 12 years old(83.46%).Gather events were evenly distributed in all grades,but outbreak often occurred in grade six(50.90%).Delayed report and more classes affected led to longer duration of outbreaks.In addition,5 outbreaks related to gather activities.All the cases had the rash and 48.90%of cases had fever.The cases who first appearred a rash or a rash along with fever accounted for 73.52%.Rash first appeared in head and neck and trunk among 83.48%of cases.The break through cases accounted for 67.80%,and growth year by year.3.The varicella cases'cost per capita was 430 yuan in four varicella outbreaks.According to the annual varicella cases in Tianjin(7361),the annual economic burden was 3.1652 million yuan.The economic loss was 13906 yuan in 4 outbreaks.326person-hours were put in and 1705 days'courses were missed in these outbreaks.Owing to handle properly,469 incident cases,4690 days'course delay,1407 days'work delay and 0.2018 million yuan economic burden were avoided.Cost-benefit ratio was 1:14.5 duing to disposal of outbreak.Conclusion1.From 2010 to 2013,the annual morbidity of varicella was declining and the break through cases were growing annually.The cases aged<15 years old accounted for68.44%.The incidence increased with age from 1 to 15 years old,then decreased.2.The varicella events presented two peaks in spring and winter in Hedong District and had highest incidence in November,which need to attention.Most events occurred in primary school and presented school aggregation.The main factors in events spread was delayed report,which led to delay of the CDC professionals'surveying and disposing.3.The number of cases and economic losses would be reduced by disposing the outbreak timely.
