Objectiv:Investigate the prognostic factors of medullary thyroid carcinoma?MTC?.Methods:In this study,we retrospectively analyzed the prognostic factors of 91 patients with medullary thyroid carcinoma from January 2005 to November 2016 in the First Affiliated Hospital of China Medical University,by using statistical single factor and multiple factors method.Including 40 males and 51 females,the age interval of 22 to 77years.13 cases of palpable and bilateral ipsilateral or bilateral cervical lymph nodes.The operative range included partial thyroidectomy in 11 cases?12.8%?,lobectomy and central lymph node dissection in 26 cases?30.2%?.17 cases?19.8%?of total thyroidectomy plus lymph node dissection in central area,32 cases?37.2%?of total thyroidectomy plus central area lymph node dissection,and 3 patients received I131after operation.Results:All the histopathological findings were medullary carcinoma and the lymph node metastasis rate was 65.3%.The 5-and 10-year survival rates were 96.3%and92.4%,respectively.Univariate analysis showed that gender,age,primary tumor size,distant metastasis,postoperative calcitonin levels the impact of the shadow of the independent risk factors after healing.Multivariate analysis showed that distantmetastasis is an independent prognostic factor affectingsurvival of patients.In our survey,many postoperative patients survived,despite their postoperativemaintenance of elevated calcitonin levels.Conclusion:distant metastasis is an important factor in the prognosis of medullary carcinoma,often suggesting poor prognosis or increased likelihood of recurrence.Monitoring the level of calcitonin in medullary thyroid carcinoma prognosis of great significance for the assessment of MTC prognosis has important reference value.
