Follow-up Study About The Effect That Three Kinds Of Internal Fixation Methods For Treating The Hauck ? Ulnar Styloid Fracture

?Objective?By comparing three kinds of internal fixation methods which include double Kirschner pin fixation,double Kirschner pin combined with tension band fixation and bone anchorage fixation in the clinical efficacy of the treatment of Hauck type ? ulnar styloid fracture,providing evidence-based theory and practice basis for the clinical internal fixation choice of Hauck type ? ulnar styloid fracture fixation,and getting familiar with the methods and ideas of clinical project research through this research.?Methods?The clinical data of the surgical treatment of Hauck type ? ulnar styloid fracture from January 2012 to January 2017 in Guangzhou Medical University Affiliated Shenzhen Shajing Hospital were collected and used.24 cases who had double Kirschner pin fixation,double Kirschner pin combined with tension band fixation and bone anchorage fixation among all of the patients were selected.The followup medical records,imaging examination data,surgical records,postoperative modified Mayo score scale were retrospectively analyzed.All of the selected case data were accomplished by admission diagnosis,intraoperative treatment and postoperative follow-up by the same surgical team members.According to different surgical methods,they were divided into group A(double Kirschner pin fixation group)with 8 cases;group B(double Kirschner pin combined with tension band fixation group)with 5 cases;group C(bone anchor fixation group)with 11 cases.They were in total of 24 surgical patients.The 24 patients were followed-up for 8.75 to 18.25 months with an average of 14.52±2.07 months.They all had complete postoperative follow-up review records.The modified Mayo wrist score scale was used to measure wrist grip strength,pain,functional status,and limb extremities of patients in order to grade the function of wrist joint.All cases were closed fractures.The surgical operation,age,gender,time of intraoperative processus styloideus ulnae fixation,blood loss of intraoperative processus styloideus ulnae,postoperative follow-up time,fracture healing time,rehabilitation intervention time,postoperative follow-up wrist function scores were performed on all medical records and they were made a statistical analysis to compare the comprehensive effects of the three methods.?Results?The differences in the age,gender,time of intraoperative processus styloideus ulnae fixation,blood loss of intraoperative processus styloideus ulnae and postoperative follow-up time among three groups were not statistically significant after the statistical test(P> 0.05),there was comparability in the three groups;there was a statistically significant difference among the three groups in fracture healing time,modified Mayo wrist function score,and postoperative rehabilitation intervention time(P<0.05).After the comparison between every two groups,the postoperative fracture healing time,the last follow-up which was based on the modified Mayo wrist function score and the postoperative rehabilitation intervention time of the bone anchorage fixation group were all superior to the double Kirschner pin combined with tension band fixation group and the double Kirschner pin fixation group.The fracture healing time and the functional rehabilitation intervention time before and after the operation between the double Kirschner pin fixation group and the double Kirschner pin combined with tension band fixation group were no significant difference.?Conclusion?The above three internal fixation methods are all effective for the treatment of Hauck type ? ulnar styloid fracture.The bone anchorage fixation group is superior to the double Kirschner pin fixation group and the double Kirschner pin combined with tension band fixation group in terms of postoperative fracture healing time,postoperative wrist function score,and postoperative rehabilitation intervention time.The Hauck type ? ulnar styloid fracture is recommended to be fixed with bone anchors.
