Comparison Of Curative Effect Between Medial Malleolus Fracture Treated With Titanium Metal Tension Band And Kirschner Wire Tension Band

Objective: To compare the clinical effect of perforated metal pin titanium cable tension band and Kirschner wire tension band in the treatment of medial malleolus fracture.Methods: A total of 63 patients with closed internal and external malleolar fractures(spin-external rotation)who had complete data from January 2013 to December 2015 in our hospital were analyzed.Sixty-three cases of internal and external hernia fractures were divided into two groups according to the internal hemorrhoids treatment method: in group A,there were 32 patients in total.The metal banded needles were fixed with a tension band of titanium cables.In group B,there were 31 patients.The Kirschner wire was used for tension band fixation;Titanium plate and screw fixation were unified for titanium fractures(titanium plates and screws were produced by the same manufacturer).Both groups of patients were treated by the same physician group,and there was no difference in surgical skills and treatment.Observe and record the average age,gender,degree of ankle swelling on the day of surgery,intraoperative blood loss,length of internal hernia incision,operation time,clinical healing time of medial malleolus fractures,and postoperative internal hemorrhoids.Whether the fracture,the tension band was removed,whether the Kirschner wire or holey metal bone needle was loosened and the comfort zone of the postoperative surgery area was satisfactory,and the effect of the postoperative ankle joint was evaluated according to the AOFAS ankle function score,and the postoperative internal hemorrhoid was evaluated.Incidence of complications,incidence of pain,and comfort.Results:All 63 patients were followed up for 12-18 months.There was no statistical difference between the average age,gender,and degree of swelling of the affected ankle in the two groups on the day before surgery(P>0.05).There was no significant difference in the incision length,intraoperative blood loss,operation time,and clinical healing time of medial malleolus fracture between the two groups(P>0.05).Postoperative complications of internal hemorrhoids: No complications in group A,incidence of complications of internal hemorrhoids was 0%;2 cases of loosening of Kirschner wire in B group,1 case of tension band rupture,1 case of tension band detachment,no internal In the case of punctured skin with fixed objects,the incidence of complications of internal hemorrhoids was 12.9%;the incidence of postoperative complications in group A was significantly smaller than that in group B(P<0.05).The AOFAS ankle joint function score at 12 to 18 months after operation: 28 cases in group A were excellent,2 cases were good,2 cases were moderate,and the excellent and good rate was 93.8%;25 cases were excellent in group B,and 4 cases were good.In 2 cases,the excellent rate was 93.5%;there was no significant difference in the good rate between the two groups(P>0.05).The incidence of postoperative internal hemorrhoid pain: There was no pain in 32 patients in group A,and the incidence of postoperative internal hemorrhoid pain was 0%;in group B,there were 27 cases of no pain,3 cases of mild pain,1 case of moderate pain,and 0 cases of severe pain.The incidence of posterior medial malleolus pain was 12.9%.The incidence of postoperative medial malleolus pain was significantly lower in group A than in group B(P<0.05).Postoperative comfort: No pain in group A,2 cases of foreign body sensation,no discomfort in 30 cases,excellent rate of 93.8%;4 cases of pain in group B,5 cases of foreign body sensation,no discomfort in 22 cases,excellent rate was 71.0%;A The postoperative comfort of the group was significantly better than that of the group B(P<0.05).Conclusion:Compared with the traditional Kirschner wire tension band,the tensioned band of the metal needle with a hole has the advantages of avoiding the loosening of the bone needle,avoiding the tension band from being dislodged or broken,and irritating the soft tissue in the operating area.It is a clinical treatment for internal hemorrhoids.A safe,reliable and effective internal fixation method for fractures.
