The Effects Of Tripterygium On C1GALT1/Cosmc In Patients With Purpura Nephritis

Objective:By observing the Tripterygium wilfordii glycosides tablets(Tripterygium wilfordii polyglycoside,TW)in treatment of allergic purpura nephritis(HSPN)curative effect,and the serum core 1 beta 3-galactose transferase(beta 1,3-galactosyltransferase,C1GALT1)and its partner protein Cosmc influence,to explore the diagnosis and treatment TW HSPN mechanism,provide the basis for clinical application.Methods:Using single blind random control method,will be included in the standard distribution of 60 patients according to the proportion of 1:1 to tripterygium wilfordii glycosides tablets treatment group(TW),routine treatment group(control group),30 cases in each group.Two groups all use allergy,anticoagulation,Angiotensin II Receptor blockers,Angiotensin Receptor Blocker,ARB)foundation treatment,such as TW group added with TW(specifications 10 mg * 50 pieces,jiangsu pr newswire pharmaceutical co.,LTD,approval number: Z32021007)approved by the state,usage: TW by routine dose of 1mg/kg · d,oral three times.The data were recorded in the case observation table(CRF)after treatment for 0 days(before treatment)and 28 days after treatment(4 weeks after treatment).SPSS22.0 statistical software was used for statistical analysis of the data,and the efficacy indexes were compared between the two groups before and after treatment:24h urine protein quantification,urine RBC count,blood C1GALT1,Cosmc,etc.Safety index blood: white blood cell count(WBC),platelet count(PLT),alanine transaminase(ALT),blood creatinine(Cr)and other changes.Results:1.Age and gender had no effect on the study(all P were >0.05).2.There was no statistically significant difference between the two groups in protein quantification,erythrocyte count,blood C1GALT1 and Cosmc 24 hours before treatment(all P were >0.05).3.Comparison of protein quantification,urinary RBC count,blood C1GALT1 andCosmc in the TW group at 24 hours before and after treatment was statistically significant(all P <0.05).4.Quantitative comparison of C1GALT1,Cosmc and 24-hour protein before and after treatment in the control group,all P <0.05,with statistically significant difference.Comparison of erythrocyte count in urine,P>0.05,showed no statistically significant difference.5.Comparison of C1GALT1,Cosmc,24-hour protein quantification and urinary RBC count between the two groups after treatment,P <0.05,and the difference was statistically significant.6.The total effective rate of TW group was 96.7% and that of control group was 80.0%.TW group had better curative effect than control group.7.Compared with adverse reactions,there was no abnormal liver and kidney function and blood routine between the two groups before and after treatment,and the difference was not statistically significant(P>0.05).Conclusion:1.TW can reduce the quantitative and urine erythrocyte count of 24 h in HSPN patients,and its mechanism may be to improve the expression level of blood C1GALT1/Cosmc,and play a therapeutic role.2.TW treatment HSPN is safe and effective,with few adverse reactions.
