| Objective:To study on the syndrome type of the Henoch-schonlein Purpura Nephritis through review of the literature analysis and lay the foundation for further exploration of syndrome differentiation ideas and the law of prescription drug.There are many reports of this disease clinical research, but there is no reported its literature analysis. Methods:By retrieving the published literature about the Henoch-schonlein Purpura Nephritis TCM syndrome types, characteristic of symptoms, Li-fa recipe in the Qinghua Tongfang, Wanfang Database and CMCC from1990to2011and classifying statistically the modern journals literature about the different syndrome types, symptoms, Narikata and self prescription of the Henoch-schonlein Purpura Nephritis, analyze and summarize the basic syndrome types, common symptoms and drug law. Results:(1)88literature documents satisfying the criteria are integrated into30TCM syndromes. The syndrome type is divided into the excess syndromes, deficiency syndromes and the mixed. Among the documents,12are excess syndromes,9deficiency syndromes and4mixed. After the merger the largest type is the type of spleen and kidney deficiency, followed by the type of wind-heat, the type of blood heat and blood stasis and the type of toxic heat. Syndromes of distribution and pathogenesis of (HSPN) Blood air, fire, blood stasis is basically the same.(2) In the18literature documents reporting the syndrome type of the Henoch-schonlein Purpura Nephritis and the study of case numbers,9are excess syndromes,6deficiency syndromes and3the mixed. The dialectical is mainly at blood heat and blood stasis, wind-heat, Yin deficiency and heat stasis, spleen and kidney deficiency syndrome type.(3)367TCM symptoms in the66literature documents are integrated and summarized according to the similar symptoms but different names and finally merged into40classes TCM Symptoms. After that, it is found that the most common symptom features are proteinuria or hematuria, skin purpura, mental fatigue, shortness of breath, fatigue and so forth.(4) In88literatures satisfying the inclusion criteria, there are75articles using classic recipe treating, a total of55, of which:clinical physicians use Xijiaodihuang decoction, Yinqiaosan, the thistle Yinzi, ZDP, the spleen decoction and so on.(5) There are87articles talking about drugs, the medication frequency from high to low sort is Rehmanniae, and Dan, red peony root, angelica, Rhizoma Imperatae, Astragalus, Salvia, comfrey, thistle and so on. Conclusions:(1) The syndrome type of this disease include excess syndrome, deficiency syndrome, the mixed syndrome of excess and deficiency, This shows that the development of disease is also the transformation process from the excess to deficiency. Among them, the early onset is excess syndrome, the basic syndrome type is wind-heat syndrome, blood stasis of blood heat, toxic heat and damp heat stasis type; Disease Interim is the mixed syndrome of excess and deficiency, the basic syndrome type is yin deficiency heat stasis; late disease is the main deficiency syndrome, the basic syndrome type are spleen and kidney deficiency, Qi and Yin Deficiency type, wang, liver and kidney Yin Deficiency.(2) The basic symptoms of the disease are proteinuria or hematuria, skin purpura, Shenpi, shortness of breath, fatigue, abdominal nausea, anorexia, fever, edema, dizziness, palpitations, red throat, throat, waist and knee pain, weakness and abnormal bowel movements. Heat, stasis, the imaginary symptoms characteristic is consistent with the syndrome type statistics results. Meanwhile, numbness, flexion and extension negative, gum swelling, sweating and other symptoms reported less in the literature, but it should be attracted by the attention of the clinician, in order to avoid misdiagnosis.(3) Statistical analysis of classic prescription, summed up the basic prescription of the clinical treatment of this disease:the basic prescription of excess syndrome is Xijiaodihuang decoction, Yinqiaosan, thistle Yinz and San Ren decoctioni; the basic prescription of deficiency syndrome is ZDP, Gui spleen decoction, Zhenwu decoction, Taohongsiwu decoction Liuweidihuangwan and Erzhiwan. Compared to the prescription statistics and drug statistics, although the result of the prescription is not exactly, but The Therapeutic efficacy are detoxification, blood circulation, replenishing qi and yin and yang, provides a guideline for clinical Herbal P rescription. |
