Objective: To explore the role of ST6 Gal NAc2,C1GALT1,Cosmc and Gd-IgA1 in the pathogenesis of IgAN,provide new therapeutic directions and entry points for IgAN prevention.Methods:Blood samples were collected from 40 IgAN patients in our hospital from January 2019 to December 2019 and 40 normal people who came to our hospital for medical examination.Patients in the IgAN group were confirmed pathologically by renal biopsy,including 22 male patients and 18 female patients,with an average age of50.34 ± 10.82 years.There were 26 males and 14 females in the normal group,with an average age of 45.57 ± 12.52 years.Patients in the IgAN group and the normal group received two 2m L venous blood samples each on an empty stomach with a vacuum tube in the morning.One was used to isolate the plasma,and the other was used to isolate peripheral blood mononuclear cell.Detection and comparison of the expression levels of ST6 Gal NAc2,C1GALT1 and its molecular chaperone Cosmc gene between the two groups of patients by RT-PCR;The correlation between Gd-IgA1 and ST6 Gal NAc2,C1GALT1,and Cosmc were detected and compared between the two groups of patients.Results:Collect the basic data of the two groups of patients.After statistical comparison,there was no significant difference in gender,age,HDL,cholesterol,urea,and C3 content between the two groups(P> 0.05).There were some patients in the IgAN group.24 h urinary protein was quantitatively increased and hematuria appeared.The control group did not.The body mass index(BMI),systolic blood pressure,diastolic blood pressure,blood creatinine,blood uric acid,triglycerides,and cystatin C in the IgAN group were higher than those in the control group,the glomerular filtration rate(e GFR),and serum albumin were lower than In the control group,the difference was significant(P <0.05).The comparison of three gene expression levels in peripheral blood mononuclear cells of the two groups of patients showed that the expression level of ST6 Gal NAc2 gene in the IgAN group was 1.27 times that of the control group,and the expression level of C1GALT1 gene in the IgAN group was 0.75 times that of the control group.IgAN The expression level of Cosmc gene in the group was 0.68 times that of the control group.There were significant differences in the analysis of geneexpression changes,P <0.05.The ELISA kit was used to check the plasma Gd-IgA1 levels of IgAN patients and healthy controls.The results of the study showed that the average Gd-IgA1 levels of IgAN patients were significantly higher than(12.3 ± 5.68?g/ m L)healthy controls(7.43 ± 4.83?g / m L),with significant difference,P <0.001.By correlation analysis of peripheral blood lymphocyte enzymes ST6 Gal NAc2,C1GALT1 and Cosmc expression in patients with IgAN and the relationship between IgA and serum Gd-IgA1,we found that patients with high levels of Gd-IgA1,their ST6 Gal NAc2(P <0.001)was significantly higher and correlated with it.It was negatively correlated with the expression of C1GALT18(P <0.001)and Cosmc(P <0.001).Conclusion:In this study,it was found that body mass index,systolic blood pressure,diastolic blood pressure,blood creatinine,blood uric acid,triglycerides,and cystatin C were higher in the IgAN group than in the control group,glomerular filtration rate(e GFR),and serum albumin.Lower than the control group.The expression of ST6 Gal NAc2 gene was up-regulated in peripheral blood mononuclear cells of patients with IgAN,and the expression of C1GALT1 and Cosmc genes was down-regulated.The average Gd-IgA1 level of IgAN patients was significantly higher than that of healthy controls.Patients with high levels of Gd-IgA1 in IgAN patients had significantly higher expression levels of ST6 Gal NAc2 genes,showing a positive correlation.However,it was negatively correlated with the expression levels of C1GALT1 and Cosm genes.The results of this study provide more data and ideas for basic research and clinical treatment of IgAN,and provide more possibilities for finding treatment methods for IgAN disease.
