Objective:Malocclusion and orthodontic treatment may have physical and psychological impact on patients.With the transformation of medical model,the evaluation of oral health-related quality of life is not only confined to the absence of oral diseases and corresponding dysfunctions,but also extends to whether oral condition has negative impact on social life and self-confidence.The purpose of this study was to compare the oral healthy-related quality of life(OHRQOL)between clear aligner and fixed appliance,and also to compare the impact on the patients'personality and severity of malocclusion.This study is to help doctors better understand the patients orthodontic experience during the treatment,and provide some advices for the patients in choosing appliances.Methods:Two hundred thirty-one adult patients were randomly enrolled,whom were treated in the Affiliated Stomatology Hospital of Kunming medical University from 2017.4 to 2018.2.There were 51 males and 162 females with the average age of 24.4.Grouping is based on appliance type,personality,and severity of malocclusion.? According to the different appliances applied,patients were devied into self-ligating brackets group and clear acligner group;? According to the Eysenck Personality Questionnaire,clear aligner treated patients were divided into three groups:introverted,intermediate and extroverted;? According to the ICON index,the clear aligner treated patients were divided into four groups:easy,relatively easy,moderate,and difficult.OHRQOL was assessed by OHIP-14 for 3 times:before treatment(TO),1 week after bonding(T1)?1 month afterbonding(T2).Repeated measurement variance and Bonfferoni tests were used to compare the relative changes of OHRQoL among the different time points.Results:The quality of life of all patients was affected by orthodontic treatment.The OHRQOL scores increased significantly and reached the peak at T1 stage and the changes between different groups was statistically significant.? Patients with different appliances had statistically significant differences except in functional disability,psychological discomfort and social disability.The clear aligner group reported lighter in both physical pain and psychological disability was than that of the self-ligating bracket grouo(p<0.01).In both groups,T1 scores were higher than those of T2,except for psychological discomfort,mental disability and handicap.?Clear aligner treated patients with different personality have shown some extent of differences except for psychological disability.The extroverted group reported lighter physical pain than other group at T1<p<0.01)The total score of introverted patients in the T1 period was 17.8±4.4,which was the highest in the whole experimental group.? Patients with different degrees of malocclusion have a certain influence on the effect of treatment on their OHRQOL,but only the difference in physical pain is statistically significant.Conclusions:Orthodontic treatment will have impact on the oral health-related quality of life for patients,the impact is related to the appliance used and patients own conditions.They all have the most extensive influence on OHRQOL during the first week of treatment.? The effect of different appliances on the OHRQOL of patients was different.Clear Aligner had less influence on the OHRQOL than self-ligating brackets.? Personality traits have different effects on patients' OHRQOL.Introverted patients are more affected than external ones,mainly in physical pain and psychological discomfort.? The severity of malocclusion has a certain influence on the OHRQOL,as the severeler malocclusion has greater impact on the OHRQOL.
