A Research Of The Elders’ Oral Health Status And Its Correlation With Oral Health-related Quality Of Life In Kunming City

Objective:This paper investigates the oral health status of the elderly people in Kunming city, researches the correlation of their oral health and quality of life, analyzes its correlation in depth at the same time. In order to provide scientific evidence for prevention of oral disease, and for improvement of oral health related quality of life in elderly people, this research was accomplished.Methods:A total of 687 elderly people with 60-88-year-old in 13 districts of Kunming were surveyed by Chinese vision of oral health impact profile (OHIP-14) and oral health examination were accomplished from September,2013 to January,2015. After this investigation, a data template was established by Epidata software. Descriptive statistics, Pearson X2 test, FisherX2 test, signed-rank test were performed to study the states.Results:A total of 687 valid forms and questionnaires were obtained, among them,359 were men,328 were women. The old people ranged from 60 to 88 years. The study is divided into two parts.In the first part, tooth examinations were used to investigate oral health condition of elderly people, including periodontal disease, dental caries, dental defect and dental restorations condition. The result is:1. Patients with advanced periodontitis in elderly people in 136 cases, accounting for 19.8%; early periodontitis in 291 elderly patients, accounted for 42.4%; periodontal healthy completely in 2 cases, only accounting for 0.3%.2. The investigation group of DMFT was 16.12; a total of 514 caries, caries prevalence rate was 74.8%; the number of root surface caries prevalence was 287, the prevalence rate is 41.8%.3. The average number of oral cavity residual teeth of 11.21 stars; dentition defect prevalence rate is 94.3%; the edentulous prevalence rate was 4.5%; dentition healthy completely in 8 cases, accounted for 1.2%.4. The proportion of older people is wearing removable partial denture is the highest, accounted for 34.9% and 33.6% respectively in the upper and lower jaw; require one or more units of the joint repair demand ratio is the highest, accounted for 28.8% and 37.1% respectively in the upper and lower jaw.The second part is oral health related quality of life (OHRQOL) in respondents through the scale OHIP-14. The result is:1. The highest score of OHIP-14 is 39, most of respondents concentrate mainly on 14-19. Average score is 12.61±8.02.2. "Affect pronunciation" is a negative impact on the max proportion (6.7%), followed by "feel worse taste" (5.3%), followed by "not satisfied with their eating"(4.4%).3. The average score of advanced periodontitis in 136 elderly patients is 15.5(10,19), average score of early periodontitis in 291 elderly patients is 14(7,18), average score of 212 gingivitis and patients with odontolith is 6(2.75,11.5) and 8(2,14) respectively. The result suggested in the 14 items of OHIP-14, the statistical significance exists in the following 4 items, "obvious pain appeared", "unsatisfied with own diet", "become easy to lose one’s temper to others", "feel life is not so satisfactory". (p<0.05)4. The average score of OHIP-14 in elderly with complete dentition is 3(0.25,5.75), elderly with dentition defect’s average score is 13(6,18), elderly with edentulous’s is 23(16,28). The result suggested in the 14 items of OHIP-14,the statistical significance exist in the following 7 items, "affect pronunciation", "feel worse taste", "feel uncomfortable when eating anything", "feel uneasy in front of other people", "unsatisfied with own diet", "have to stop when eating ", "feel life is not so satisfactory". (p<0.05)Conclusions:1. The periodontal health status of aged people in Kunming city is not good. Dental caries, missing teeth phenomenon.2. OHRQOL of aged people in Kunming city is poor. Oral disease impact quality of life mainly on "dysfunction" and "physiological barrier".3. OHRQOL of advanced periodontitis is poorer than early periodontitis, negative effects mainly concentrate on the 4 fields, "physiological pain", "physiological barrier", "human communication disorders" and "physical disability".4. OHRQOL of elderly with dentition defect is poorer than elderly with edentulous, negative effects mainly concentrate on the 5 fields, "dysfunction",", "physiological pain", "psychological discomfort", "physiological barrier", "physical disability".
