BackgroundSepsis is a life-threatening organ dysfunction caused by a hostile response to an uncontrolled host infection.Its high morbidity and mortality threaten human health,and aggravates the burden on medical and health resources.The essence of sepsis is the multiple organ dysfunction caused by the body's immune response to severe infections.The key of the dysregulated host response is not only the systemic inflammatory response,but also the immune regulation disorder,that is,the excessive immune activation accompany or developed to a wide range of immunosuppression.The body's immunosuppression makes it difficult to clear the infection,and the chance of acquiring a hospital-acquired infection increases.This not only increases the investment in medical resources,but also correlates with an increase in mortality.There are very important clinical values for the treatment of immune disorders.However,there is a dilemma in the choice of intervention time point for immunomodulators and a worry that a single target may aggravate the disorders of inflammatory.Traditional Chinese medicine has advantages in stabilizing the body in multiple stages and at multiple targets and is worthy of research and development.The clinical manifestations of sepsis are complex.Syndrome differentiation based on yin and yang can simplify it.ObjectiveTo observe whether Sini Decoction can improve the prognosis and immune disorders of patients with sepsis sepsis,and provide some medical evidence of treating sepsis and its Immunosuppression with traditional Chinese medicine MethodsThis study was a prospective,randomized,controlled clinical study.Patients who meet the sepsis criteria were received from the Intensive Care Unit of Guangdong Provincial Hospital of Traditional Chinese Medicine fromMay 2016 to December 2017.These paients were randomly divided into 2 groups,study group and control group.The control group was given conventional medicine,and the study group was given Sini decoction additionally,During the study,the patient's inflammatory biomarkers,immune indices,survival rate and so on were observed to undertake analysis and evaluation.ResultsA total of 44 people were included in this study,22 in each group.There was no significant difference in the baseline data between the two groups(P>0.05).On the fifth day after treatment,the SOFA score and LAC of the Chinese Herbal Medicine group decreased significantly(P<0.05),and the anti-inflammatory factor IL-10 decreased significantly compared with the control group.(P<0.05),there was no significant change in inflammatory markers TNF-a,IL-6,CRP,PCT,and WBC.Immunoglobulin IgA and IgM were significantly higher than before compared with the control group(P<0.05),while there was no significant change in T lymphocyte subsets,circulating lymphocytes and monocytes,and the complement system;28-day survival,total hospital stay,length of ICU stay and hospitalization costs between the two groups were no statistical difference.ConclusionSini decoction can reduce SOFA score and lactate levels,improve organ failure and tissue perfusion in patients with sepsis.It can down-regulate the expression of anti-inflammatory cytokine IL-10 in sepsis patients,increase the levels of IgA and IgM,and is related to the improvement of immunosuppression and humoral immunity,suggesting that Sini decoction has immunomodulatory effect on patients with sepsis.
