Objective:The purpose of this study was to observe the clinical manifestations and quality of life of patients with rare respiratory diseases Autoimmune Pulmonary Plveolar Proteinosis treated with Wen San Ding transdermal therapy, which was a self-made herbal medicine with the effects of Wenjing Tongluo and Xingqi Huatan.Method:From March 2017 to March 2018,this study continuously collected 5 patients diagnosed with APAP from the Peking Union Medical College Hospital. After signing the informed consent, the clinical observation of the Wen San Ding transdermal therapy was performed. The clinical observations included clinical manifestations and activities of daily living. The observations included mMRC dyspnea scores, cough severity visual analogue scores, cough volume visual analogue scales, and observations of activities of daily living including the SF-36 quality of life scale,6-minute walking test, rest and post-activity oxygen saturation. Before the start of treatment, the patient's medical history was collected and the initial evaluation of the clinical observations was performed. Then give the Wen San Ding transdermal therapy, the treatment sites were bilateral Feishu and Yushu points, once a day for 60 minutes, for a total observation period of 3 months.Detailed clinical records were performed during the period. After 3 months, the contents of clinical observations were evaluated again. Finally, the self-contrast method was used to analyze the statistical differences in clinical observations before and after treatment.Result:Basic conditions:The clinical manifestations of five patients were mainly respiratory symptoms and systemic hypoxemia symptoms;and the clinical manifestations of APAP patients might not be directly proportional to imaging and lung function results.Traditional Chinese medicine syndrome differentiation:Among the 5 patients,3 cases were identified as Tanyu Zuluo. After treatment,Obese tongue body and white thick and greasy moss were significantly reduced, symptoms of chilling were also improved;while the other 2 patients reported significantly improved asthma symptoms after climbing the stairs, and the oxygen saturation also increased significantly. However,in 1 patient with both qi and blood deficiency, the degree of symptom improvement was not significantly different in the other patients. Therefore,it also showed that Wen Sanding Transdermal treatment was more effective for Tanyu Zuluo type, especially for cold-resistance,and it could also be said that both Deficiency Syndrome might have a relatively weaker effect.Clinical manifestations:The mMRC score and cough volume VAS score after treatment were significantly improved compared with those before treatment in all patients(P?0.05),and the cough severity VAS score after treatment was lower than before treatment(P?0.05),However,their cough symptoms had improved.Daily activity status:In terms of activities of daily living, the 6MWT,resting oxygen and SF-36 scores of all patients after treatment were significantly improved cormpared with those before treatment, and there was a statistically significant difference(P?0.05).Conclusion:1.In patients with APAP who had undergone Wen Sanding Transdermal treatment, the amount of asthma and cough were significantly improved, and the symptoms of cough were also partially improved. It showed that the Wen San Ding transdermal therapy with the effects of Wenjing Tongluo and Xingqi Huatan were helpful to improve the respiratory symptoms of APAP patients;2.After APAP patients were treated with Wen Sanding Transdermal treatment, the SF-36 quality of life scale,6-minute walking distance, resting oxygen saturation, and post-activity oxygen saturation were all significantly improved. The results showed that the Wen San Ding transdermal therapy with the effects of Wenjing Tongluo and Xingqi Huatan could help to improve the oxygenation function of patients with APAP and improve their endurance and quality of life;3.Wen Sanding Transdermal treatment was helpful for the treatment of APAP,it might be a new type of Chinese medicine therapy for the treatment of rare APAP.
