This Study On The Correlation Of TCM Syndrome Element In Patients With Advanced Colorectal Cancer Before And After Chemotherapy

ObjectiveTo investigate the distribution and evolution of TCM syndrome elements in patients with advanced colorectal cancer before and after chemotherapy,and to explore the evolution characteristics of their syndrome elements in different locations of colorectal cancer,and the correlation between the side effects of chemotherapy and the syndrome elements.It provides a reference for Chinese and Western medicine in the treatment of patients with advanced colorectal cancer,so that Chinese medicine can play a role in the adjuvant treatment of tumor patients,in order to better enhance the curative effect and improve the quality of life of patients.Using chi square test and other statistical methods to compare before and after chemotherapy,to get a statistical significance and clinical significance of the index.MethodThe prospective,questionnaire survey was performed to collect 60 patients who were hospitalized in our hospital from March 2017 to December 2017 and pathologically diagnosed as stage IV colorectal cancer.According to the questionnaire,collect the general information,TCM syndrome information of patients before and after chemotherapy and the side effects.According to Zhu Wenfeng's " Syndrome Element Differentiation",draw the patient's syndrome diagnosis,and then build all the information into a database of information,SPSS17.0 software for data analysis.60 cases of general patients before and after chemotherapy were summarized,including TCM syndromes,disease syndrome elements,disease location elements.And analysis the relationship of side effectsand syndrome elements.According to the knowledge of basic theory of TCM,diagnostics of traditional Chinese medicine and internal medicine of TCM,we analyzed the essence of TCM syndrome changes before and after chemotherapy,so as to provide evidence for TCM treatment of left and right colorectal cancer patients before and after chemotherapy.Results60 cases of colorectal cancer patients before chemotherapy appeared in the main TCM syndromes are white tongue fur,abdominal pain,abdominal distension,taut pulse,slippery pulse,constipation,fatigue,frequent defecation,uncomfortable tired greasy and nausea,the disease syndrome elements are dampness,qi deficiency,qi stagnation,blood stasis,blood deficiency,yang deficiency,heat(fire),phlegm,the disease location elements are colon,spleen,liver,kidney,stomach,bladder,lung.The main TCM syndrome after chemotherapy for lassitude,tongue white,nausea,abdominal pain,constipation,defecation,wiry often uncomfortable,dark purple tongue,tired greasy,the disease syndrome of qi deficiency,qi stagnation,blood stasis,blood deficiency,dampness and yang deficiency,and the disease location elements of spleen,kidney,liver,stomach,lung.According to Chi square test,syndrome and syndrome elements with statistical difference are abdominal pain,tiredness,fatigue,nausea,dizziness,slippery pulse,qi deficiency,qi stagnation,blood stasis,blood deficiency,spleen and kidney.The left colon cancer patients before chemotherapy with the main TCM syndromes for white tongue fur,slippery pulse,abdominal pain,abdominal distension,defecation etc,the disease syndrome element of dampness,blood stasis,qi stagnation,qi deficiency,fire(heat),and the disease location elements of colon,spleen,liver and gallbladder.The main TCM syndrome after chemotherapy for pulse string,lassitude,defecation,nausea,abdominal distension,abdominal pain and discomfort,dark purple tongue,tired greasy,pain refused to press,often constipation,disease syndrome of stagnation of qi,blood stasis,dampness,qi deficiency,syndrome factor for colon,spleen,liver and stomach kidney.According to the Fisher exact probability test,the syndrome and syndrome elements with statistical difference are white tongue fur,vein slippery,tired lassitude,pulse chord,qi stagnation and blood stasis.The correlationbetween syndrome elements and toxic side effects: syndrome qi deficiency,spleen are related to HGB,syndrome of qi stagnation and large intestine are related to constipation,syndrome element liver is related to nausea and vomiting,and qi deficiency,blood stasis and spleen are related to neurotoxicity.The right colon cancer patients before chemotherapy with the main TCM syndromes for white tongue fur,wiry pulse,abdominal pain,pale tongue,often eat less bloating,etc,the disease syndrome element of yang deficiency,qi deficiency,blood deficiency,qi stagnation,phlegm,and the disease location elements of spleen,liver,kidney,stomach,lung,large intestine.The main TCM syndrome after chemotherapy for white tongue fur,fatigue,nausea,fine pulse,weak pulse,long ruler eating less,abdominal pain,nocturnal enuresis,abdominal distension,pale tongue,disease syndrome of qi deficiency,blood deficiency,yang deficiency,qi stagnation,blood stasis,syndrome factor for spleen,kidney,stomach,liver and lung.According to the Fisher exact probability test,the syndrome and syndrome elements with statistical difference are fatigue,fatigue,nausea and pulse string,qi deficiency,blood deficiency,spleen.The correlation between syndrome elements and toxic side effects:syndrome elements,qi deficiency,blood deficiency and spleen are related to HGB.The syndrome elements,liver and stomach,are related to nausea and vomiting.The syndrome elements,qi deficiency,blood stasis and spleen are related to neurotoxicity.Conclusion1.The TCM Syndromes of advanced colorectal cancer with multiple deficiency and solid inclusion.In the right half colon cancer patients,deficiency syndrome is mostly seen,mainly involving the spleen,liver and kidney,and the left side colorectal cancer is mostly positive,mainly involving the viscera,the large intestine,the spleen and the liver.After chemotherapy,the syndrome element of deficiency,blood deficiency and spleen in the right half colon cancer patients are obviously increased compared with those before chemotherapy,and the traditional Chinese medicine is used to supplement qi,nourish blood and strengthen the spleen as a method.The syndrome element of qi and blood stasis in left colorectal cancer patients is increased compared with thatbefore chemotherapy.The treatment of traditional Chinese medicine is combined with the method of activating qi and blood.2.There is a certain correlation between the different side effects of left and right colorectal cancer and syndrome factors.In clinic,the side effects of chemotherapy can be alleviated by corresponding treatment.
