Clinical Study On The Changes Of Syndromes Elements Before And After Chemotherapy In Advanced Colorectal Cancer

Objective1.To sum up the changes of TCM syndromes before and after chemotherapy in colorectal cancer,and to explore the difference of syndromes caused by different chemotherapy regimens,and provide reference for TCM syndrome differentiation and treatment of advanced colorectal cancer.2.To explore the pathological changes of TCM caused by different chemotherapy regimens of colorectal cancer,and to conduct preliminary TCM analysis on the cold and heat bias,efficacy and side effects of chemotherapy,and to guide Chinese medicine to prevent and treat adverse reactions of chemotherapy.MethodsA prospective study was conducted to collect 86 patients with advanced colorectal cancer who underwent XELOX or FOLFIRI chemotherapy between February 1,2018 and December 31,2018.The symptoms,signs,and tongues of the patients before and after chemotherapy were collected.Using the method of extracting syndrome by Professor Zhu Wenfeng to extract Main syndrome elements about 45 patients with XELOX chemotherapy,41 patients with FOLFIRI regimen,and all 86 patients with chemotherapy.The chi-square test was used to analyze the statistically significant factors before and after chemotherapy.Through the change of the syndrome,the preliminary TCM analysis of the cold and heat bias,efficacy and side effects of XELOX and FOLFIRI programs was carried out.Result1.The main syndromes of 86 patients with advanced colorectal cancer before chemotherapy were large intestine,spleen,kidney and liver.The syndromes were wet,fire,phlegm and qi deficiency.The main disease syndrome after chemotherapy is spleen,stomach,kidney,large intestine,liver,qi deficiency,blood deficiency,fire,and wet.After the chi-square test,the stomach,spleen,liver,skin,qi deficiency,blood deficiency,qistagnation increased,and the large intestine,wet,blood stasis decreased.2.45 patients with XELOX chemotherapy,the main syndrome after chemotherapy is the spleen,stomach,kidney,large intestine,qi deficiency and blood deficiency.After the chi-square test,the stomach,spleen,skin,qi deficiency,blood deficiency,qi tagnation of the diseased digits increased,and the large intestine and wetness decreased.3.41 patients with FOLFIRI regimen chemotherapy,the main syndrome after chemotherapy is spleen,stomach,large intestine,liver,kidney,the disease syndrome is fire(heat),qi deficiency,blood deficiency,and dampness.After the chi-square test,the stomach,liver,qi deficiency,blood deficiency,fire(heat)increased,the wet,phlegm,cold,The decline of yang decreased.Conclusion1.Advanced colorectal cancer is mainly caused by evil spirits before chemotherapy.The pathological products of large intestine is wet,hot,and phlegm,and there are also positive imaginary.chemotherapy mainly affects the spleen,stomach,large intestine,as a focus of qi deficiency,blood deficiency and spleen and stomach loss.So after chemotherapy,Fuzheng exorcism parallel,pay attention to nourish qi and blood,care for the spleen and stomach,restore the function of the large intestine.The decline of wet and phlegm syndrome and the shrinkage of cancer indicate that chemotherapy has the effects of dehumidifying phlegm,reducing swelling and dispersing.2.XELOX program has the effect of swelling and dispersing,dampness and turbidity,and clearing heat and dampness.The cold and heat bias of this program is not obvious,and the side effects of related chemotherapy need to be treated separately.FOLFIRI program is hot,good to eliminate swelling,blood circulation,cold and warm yang,its related side effects of chemotherapy are mostly hot,pay more attention to the use of heat.
