ObjectiveBreast cancer is the leading cause of death in women.Young breast cancer patients have poorer prognosis than those in middle-aged and elderly patients.They are more likely to have genetic abnormalities,and cancer are more aggressive.They are prone to lymph node metastasis or hematogenous metastasis,and the degree of malignancy is high.The aim of this study is to investigate the clinicopathological factors and the expression of clinical histochemical markers in young breast cancer patients,so as to provide a treatment plan for individualized treatment to improve the survival rate of young breast cancer.MethodsA retrospective analysis from 2015 to 2017 of 112 patients with breast cancer under the age of 35 in the Second Affiliated Hospital of Dalian Medical University was done.Patients election criteria: postoperative pathological results for invasive carcinoma,including invasive ductal carcinoma,invasive lobular carcinoma,papillary carcinoma,mucinous carcinoma,medullary carcinoma,squamous cell carcinoma,carcinoma simplex;the patients with complete clinical and pathological data,including pathological type,tumor size,clinical TNM stage,lymph node involvement and morphological indexes such as ER,PR,Her-2,EGFR,P53,Ki67 expression;withoutpreoperative chemotherapy,radiotherapy,endocrine and immune therapy and other anti-tumor therapy.The clinical pathological factors and the clinical commonly used histochemical marker expression were collected.And 112 cases of breast cancer patients over 35 years old were selected as control.The clinicopathological data(pathological classification,tumor size,clinical TNM stage,lymph node involvement and other morphological indexes,as well as ER,PR,Her-2,EGFR,P53,Ki67 and other protein expression)were tested by chi square test.Results89 of 112 patients went to hospital with breast lump as the first symptoms,the other 22 cases with nipple discharge or hemorrhage and 1 cases with skin and nipple change.Of the 112 patients,101(90.1%)patients have premenstrual tension syndrome,105(93.8%)patients have single tumor,and 7 patients were accompanied by other benign or cystic hyperplasia.There were 7 cases of breast cancer in the immediate family(6.25%).The two groups in invasive ductal carcinoma accounts for the majority of young patients with breast cancer;the lymph node metastasis rate had significant difference between the young and elderly patients(P=0.016);young breast cancer in stage III and IV patients were more compared with the control group(31.3% vs 19.6%,P=0.046);young breast cancer patients with ER positive expression rate was lower than that of the control group(P<0.05);Her-2(+ + +)patients were more in young breast cancer than in elderly breast cancer(P=0.04);The proportion of basal like type in young breast cancer patients was higher than in elderly patients with breast cancer(P=0.02).Luminal A and Luminal B in young breast cancer are less than in elderly patients(P=0.04).ConclusionBreast cancer in young patients are more aggressive and has poorer prognosis than in middle and elderly age breast cancer patients.
