Objective To analysis the clinical and pathological characteristics of colorectal cancer(CRC)in the elderly patients,investigate the experience of diagnosis and treatment of the elderly CRC.Methods A total number of 1343 patients with CRC from 2010 to 2012 in our hospital were included in this retrospective study.Patients were divided into the old group(OG?60 years,537cases)and young group(YG<60 years,807cases).Single factor analysis approach were adopted to compared the differences of gender,ethnicity,duration,length of hospital stay,clinical manifestations,co-morbidity,biochemical tests,tumor markers,Pathological data,surgical situation in the tow groups.Results The study included 1343 patients(OG,537/1343,40.0%,male to female ratio was 1.87:1;YG,807/1343,60.0%,male to female ratio was 1.70:1).There were statistical differences in ethnicity between the OG and YG(Han proportion were 76.9%and 61.9%,P<0.05;Zhuang proportion were 21.0%and 35.1%,P<0.05).Compared to YG,OG were more proportion of disease duration less than 3 mouths(55.1%,39.0%,P<0.05).Blood in stool(55.0%),abdomin al pain(52.2%),change in bowel habits(30.1%),stool changes(24.9%),abdominal distension(29.5%)were the main clinical manifestations in the OG.Hypertension(31.2%),diabetes mellitus(12.4%),cerebral infarction(4.9%),pneumonia(10.6%),intestinal polyps(25.0%)were more common co-morbidity in the OG,but the YG were with fatty liver(13.7%),hepa titis B(11.0%),hemorrhoids(11.0%),intestinal polyps(26.1%)more com mon.OG were less proportion of BMI)24 than YG(25.3%,32.9%,P<0.05).OG more likely to have longer hospital stay(25.5±13.2 days,23.7±11.5 days,P<0.05).OG show a higher CEA positive(55.8%,41.1%,P<0.05),lower HB.(108.2g/L,111.5g/L,P<0.05)and lower ALB(34.4g/L,36.3g/L,P<0.05)compared to YG.OG were less rectum cancer(30.7%,43.9%,P<0.05),more Sigmoid colon cancer(27.4%,16.7%,P<0.05)compared to YG.OG were more adenocarcinomas(86.8%,81.8%,P<0.05),less mucinous colorectal adeno carcinomas(6.2%,12.3%,P<0.05)compared to YG.OG were more dist ant metastasis(26.1%,20.9%,P=0.016)than the YG.OG were less likely to receive surgery(92.2%,97.1%,P<0.05)an d more-likely to receive postoperative complications(6.3%,1.9%,P<0.05)compared to the YG,included 9 cases of lung infection,7 cases of mult iple organ failure,5 cases of wound infection,4 cases of acute peritonitis,3 cases of wound healing,3 cases of anastomotic leakage,one case of postoperative intestinal obstruction,one case of arrhythmia,one case of di gestion bleeding.There was no difference in peri-operative deaths between the OG and the YG(1.3%,0.5%,P>0.05).Conclusions There were more Zhuang ethnicity in OG CRC compared to the YG.There was a shorter duration in the OG.Bellyache,bloating,intestinal obstruction,abdominal tenderness,anemia were the more common Clinical manifestations in the OG.Hypertension,diabetes,cerebral infarction,pneumonia,intestinal polyps were more common co-morbidity in OG compared to YG.OG show a higher CEA,lower HB and ALB,less proportion of BMI?24,longer hospital stay.OG were more rectum cancer and Sigmoid colon cancer,more adenocarcinomas,less mucinous colorectal adenocarcinomas,more distant metastasis,less likely to receive surgery and more postoperative complications compared to the YG.
