Objective:To analyze the effect and significance of colorectal cancer screening in the elderly and to analyze the main factors that affected the colorectal cancer screening in the elderly.Methods:A retrospective analysis of the effect and impact of colorectal cancer screening was performed in over 60-year-old elderly that had a physical examination in Center of Healthy Care for Cadres,Shanghai Ceneral Hospital from September 2011 to October 2015.A questionnaire about knowledge of those who accepted colorectal cancer screening and unaccepted or unfinished the screening was also needed.Results:Screening for the total number of 869 people,whose average age were 80.06±8.70 and the average rate of participating in 5-year-screening was 87.79%.The average screening rate of rectal examination was 59.38%and the positive rate was 32.21%.The average screening rate of first FOBT was 73.10%and the positive rate was 7.08%.The average screening rate of second FOBT was 35.17%and the positive rate was 28.97%.The average screening rate of tumor markers was 99.56%and the positive rate was 5.91%.The average screening rate of colonoscopy was 19.48%and the positive rate was 78.00%.10 colorectal cancers,8 adenomas and 6 other diseases including Melanosis coli were detected in the screening by colonoscopy.422 people were involved in the questionnaire whose response rate was 48.56%.About 199 people basically completed the 5-year-screening.Either the group that completed the screening(completed group)or the group that unfinished the screening(unfinished group)had known a little about screening methods,especially the latter(P<0.05).The unfinished group was more unwilling than the completed group in accepting rectal examination annually(P<0.05).'It is unnecessary' was the common selected reason that people in two groups did not want to accept rectal examination.The unfinished group was more unwilling than the completed group in accepting FOBT annually(P<0.05).'It might worry me' was the first selected reason in completed group,and 'It is unnecessary' was the first selected reason in unfinished group.All the people in two groups were willing to accept the examination of tumor markers annually.There were more than half of the people that were not willing to accept the colonoscopy in both two groups.No difference was found between two groups(P=0.292).Too old to accomplish screening colonoscopy' was the common selected reason in both two groups.Conclusions:Initial screening through FOBT with further screening colonoscopy should be recommended as the preferred option for colorectal cancer screening in elderly annually.It is necessary to enhance the propaganda and education on the hygienic knowledge of colorectal cancer screening in the elderly.
